摘 要:目的探讨超声波对不完全性脊髓损伤后神经源性膀胱患者的排尿和储尿功能的影响。方法前瞻性选择2020年1月至2021年12月在本院治疗的132例不完全性脊髓损伤后神经源性膀胱患者为研究对象,根据随机数字表法将患者分为研究组和对照组,每组各66例。对照组给予常规治疗,研究组给予常规治疗联合超声波。分析比较两组患者的排尿日记、尿流动力学、膀胱功能积分、生活质量及并发症。结果研究组治疗后的漏尿次数、导尿次数低于对照组,而排尿次数和单次排尿量多于对照组,差异均有统计学意义(均P<0.05)。研究组治疗后的膀胱初感觉尿量、最大膀胱容量和生活质量评分高于对照组,而残余尿量、膀胱功能积分明显少于对照组,差异均有统计学意义(均P<0.05)。治疗后,研究组的总并发症发生率低于对照组[9.09%(6/66)vs.25.76%(17/66),χ^(2)=6.371,P<0.05]。结论超声波可促进脊髓损伤后神经源性膀胱患者的排尿和储尿功能恢复,改善膀胱功能,提高生活质量,降低并发症,是一种无创、安全的新疗法。Objective To explore the effect of ultrasound on the urination and storage functions of patients with neurogenic bladder after incomplete spinal cord injury.Methods A prospective analysis was performed on 132 patients with neurogenic bladder after incomplete spinal cord injury treated in our hospital from January 2020 to December 2021.The patients were randomly divided into a study group and a control group,with 66 cases in each group.The control group received conventional treatment,and the study group received conventional treatment combined with ultrasound.The voiding diary,urodynamic parameters,bladder function scores,quality of life,and complications were analyzed and compared between the two groups.Results The number of leakages and catheterizations in the study group after treatment was lower than that in the control group,while the number of urinations and the single urination volume were higher than those in the control group,and the differences were statistically significant(all P<0.05).The initial sense of bladder volume,maximum bladder capacity,and quality of life score in the study group after treatment were higher than those in the control group,while the residual urine volume and bladder function score were significantly lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the total incidence of complications in the study group was lower than that in the control group[9.09%(6/66)vs.25.76%(17/66),χ^(2)=6.371,P<0.05].Conclusions Ultrasound promotes the recovery of urination and storage functions in patients with neurogenic bladder after spinal cord injury,improves bladder function,and improves the quality of life and reduces complications.It is a non-invasive and safe new treatment.
