数字化X线摄影技术系统胸部摄影在肺结核筛查中的临床应用

摘　　要：目的:分析数字化X线摄影技术(DR)系统胸部摄影在肺结核筛查中的应用价值。方法:回顾性分析2023年6—12月于三都水族自治县都江镇中心卫生院接受肺结核筛查居民的基本资料,共200例,患者均做DR系统胸部摄影、普通胸部平片检查。比较DR系统胸部摄影与普通胸部平片的病灶检出率。同时,将痰培养结果作为“金标准”,比较DR系统胸部摄影、普通胸部平片检查的诊断效能。结果:200例居民均做普通胸部平片、DR系统胸部摄影检查,结果发现,普通胸部平片共检出40例肺结核患者,检出率为20%,DR系统胸部摄影共检出60例肺结核患者,检出率为30%。DR影像处理前病灶(支气管扩张、支气管播散、病灶内小钙化、胸膜增厚、肺门淋巴结肿大、胸腔积液)检出率低于处理后,差异有统计学意义(P<0.05)。200例疑似肺隔离症患者的痰菌培养结果中,阳性80例、阴性120例,普通胸部平片检查、DR系统胸部摄影检查诊断中,阳性分别40例、60例,阴性分别为160例、140例。DR系统胸部摄影检查的准确度、灵敏度、特异度高于普通胸部平片检查,差异有统计学意义(P<0.05)。结论:在肺结核筛查中应用DR系统胸部摄影,能提高病灶检出率,其可以作为肺结核筛查的主要影像学技术之一。DR系统胸部摄影技术的图像处理功能强大,且操作简便。Objective:To analyze the application value of digital radiography(DR)system chest photography in tuberculosis screening.Methods:Retrospective analysis of the basic data of the residents who received tuberculosis screening in Sandu Shui Autonomous County Dujiang Township Central Health Hospital from June to December 2023,a total of 200 cases,the patients were examined by DR system chest photography and plain chest radiographs.The detection rate of lesions on DR system chest photography and plain chest radiographs was compared.At the same time,sputum culture results were used as the"gold standard"to compare the diagnostic efficacy of DR chest photography and plain chest radiographs.Results:Two hundred residents were examined by plain chest radiographs and DR chest photography.The results showed that forty patients with tuberculosis were detected by plain chest radiographs,with a detection rate of 20%,and sixty patients with tuberculosis were detected by DR chest photography,with a detection rate of 30%;the detection rate of lesions(bronchodilatation,bronchial dissemination,small calcifications within lesions,pleural thickening,hilar lymph node enlargement,pleural effusion)was lower before DR image processing than after processing,and the difference was statistically significant(P<0.05);among the surgical pathology results of two hundred patients with suspected pulmonary isolation,eighty cases were positive and one hundred and twenty cases were negative,and among the diagnosis of plain chest radiography and DR system chest photography examination,forty cases and sixty cases were positive and one hundred and sixty cases were negative,respectively;the accuracy,sensitivity and specificity of DR system chest photography examination were higher than that of ordinary chest plain film examination,and the difference was statistically significant(P<0.05).Conclusion:The application of DR system chest radiography in tuberculosis screening can improve the detection rate of lesions,and it can be used as one of the main imaging t
