摘　　要：介绍采用“解毒排脓法”的治法,采取内外兼顾的方式治疗鼻渊临床验案。毛得宏教授行医40余载,认为鼻渊的关键病机是脏腑热毒,主要病理产物是脓毒,所以自创治疗鼻渊的经验方,全方都以“解毒排脓”为核心思想,所有药物都各自发挥功效,围绕着君药解毒排脓,共同发挥清热解毒、排脓通窍的功效。该方经20余年的临床观察及基础研究,证实其可改善患者鼻部症状、体征及生活质量,且无明显不良反应。结合中药鼻腔给药的优点,采用鱼酱排毒合剂冲洗鼻腔以治疗实热型鼻渊,这种方法不仅可以使药物直接作用于病灶部位,见效快的同时避免肝脏首过作用,还可避免因其苦寒之品而受损脾胃之弊。并举验案1则介绍如下,以飨同道。This paper introduces the clinical cases of treating nasal sinusitis by adopting the method of"detoxification and pus discharge"and taking both internal and external factors into consideration.Professor Mao Dehong has been practicing medicine for more than 40 years.He believes that the key pathogenesis of nasal sinusitis is heat and toxicity in the internal organs,and the main pathological product is pus.Therefore,he created a prescription for treating nasal sinusitis.The whole prescription is based on the core idea of"detoxification and pus discharge".All drugs play their own roles,focusing on the detoxification and pus discharge of the main drug,and jointly play the role of clearing away heat,detoxification,pus discharge and orifices.After more than 20 years of clinical observation and basic research,this prescription has been confirmed to improve patients'nasal symptoms,signs and quality of life,and has no obvious adverse reactions.Combining the advantages of nasal administration of traditional Chinese medicine,fish sauce detoxification mixture is used to rinse the nasal cavity to treat nasal sinusitis of real heat type.This method can not only make the drug act directly on the lesion site,but also quickly avoid the first-pass effect of the liver,and avoid the disadvantages of damaging the spleen and stomach due to its bitter and cold nature.One case is introduced as follows for the benefit of colleagues.
