摘　　要：白癜风易诊难治,是皮肤科的疑难杂病,发病率呈逐年上升的趋势。当前研究认为免疫学说在白癜风的发病机制占据重要地位,特别是辅助性T细胞17(Th17)和调节性T细胞(Treg)间的免疫失衡,已成为近些年的研究热点之一。汗法作为八法之首,可以调和营卫,开泄肌腠,常作为皮肤疾病的治疗法则。古代典籍中亦有白为寒的论述,说明白癜风的治疗可从寒论治。现代研究认为热刺激能够改善血液运行,促进局部微循环,增强酪氨酸酶的活性,从而提高白癜风患者皮损部位的复色率。文章从Th17/Treg细胞失衡的角度探讨了温阳发汗法治疗白癜风,以期为临床治疗提供理论依据与参考。Vitiligo is easy to diagnose and difficult to treat.It is a difficult disease in dermatology,and its incidence is increasing year by year.Current studies believe that immune theory plays an important role in the pathogenesis of vitiligo,especially the immune imbalance between helper T cell 17(Th17)and regulatory T cell(Treg),which has become one of the research hotspots in recent years.As the first of the eight methods,the sweating method can harmonize the health and nourishment,and release the muscle barrier.It is often used as a treatment method for skin diseases.There is also a discussion in ancient classics that white related symptoms indicating the cold,indicating that the treatment of vitiligo can be based on the theory of cold.Modern research suggests that thermal stimulation can improve blood circulation,promote local microcirculation and enhance tyrosinase activity,and thereby increase the color recovery rate of skin lesions in vitiligo patients.The article explored the treatment of vitiligo with warming Yang to induce sweating method from the perspective of Th17/Treg cell imbalance,in order to provide theoretical basis and reference for clinical treatment.
