金实“易化”辨治思维在内科杂难病候中的运用

摘　　要：临床杂病、顽症的论治体系纷繁,如何基于庞杂的论治体系,迅速理清辨治思绪是临证过程中常面对的难题。金实教授了然百家经典,据于长期实践及反复凝练,金老为各病候的论治构建了相应的“剖析式”临证思维指南,从理、法、方层面“易化”内科杂难病候。该文则从“泄泻”“狐惑”“大偻”三大问题,窥探金老“易化”辨治思维在杂难病候中的运用。金老认为,于灵变的中医思哲中,积淀量变以洞穿主线,此理之“易”也;立于病候之主线,结合旁流或变化之论,制定“主体策略结合翼展方案”方针,此法之“易”也;针对具体病候主线,定义主干方药,结合其变化,赋予翼展之品,此方之“易”也。理、法、方之“易化”,临证思绪自然分明矣。The treatment system of clinical miscellaneous diseases and intractable diseases is much complicated.How to rapidly clarify the thoughts of syndrome differentiation and treatment based on the complicated system is a difficult problem usually occurred in the clinical process.Professor JIN Shi is familiar with hundreds of classics,and in basis of the long-term practice and repeated refinement,Professor JIN Shi constructs the corresponding“analyzing”clinical thinking guide for the treatment of various diseases,thereby simplifying various clinical miscellaneous and difficult diseases from theory,method and prescription.Here,it illustrated the application of“simplifying”syndrome differentiation thinking of Professor JIN Shi miscellaneous and difficult diseases by analyzing“diarrhea”,“Huyu disease”and“Dalou”.Professor JIN Shi suggested that when studying the traditional Chinese medicine philosophy with flexible change feature,acquiring the main line through the accumulation of quantitative change is the“simplifying”of theory.In line with the main line of the disease,it combined with side flows or changes of the theory and formulated the“main strategy combined with wingspan plan”policy is called the“simplifying”in methods.Defining the main prescription according to the main line of one specific disease and allocating supplementary herbs based on individual features is the“simplifying”in recipes.The“simplifying”in theory,method and recipe makes the clinical thoughts naturally and clearly.
