消瘀化石汤联合体外冲击波碎石术对输尿管结石患者结石排出和血清炎症因子的影响

摘　　要：目的 探讨消瘀化石汤联合体外冲击波碎石术对输尿管结石患者结石排出和血清炎症因子的影响。方法 回顾性分析80例输尿管结石患者的临床资料,将实施单纯体外冲击波碎石术治疗的40例分为对照组,实施消瘀化石汤+体外冲击波碎石术治疗的40例分为观察组。治疗14 d并完成6个月随访。比较两组临床疗效、中医证候积分、治疗前后血清炎症因子[C反应蛋白(C-reactive protein, CRP)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)及白细胞介素-6(interleukin-6,IL-6)]及肾功能[血肌酐(serum creatinine, Scr)、微量白蛋白(microalbumin, mALB)及β2微球蛋白(β2-microglobulin, β2-MG)]的变化,并记录治疗期间并发症情况;随访6个月,比较两组复发情况。结果 观察组临床总有效率高于对照组(P<0.05)。治疗后,两组中医证候积分各维度分值均降低(P<0.05),且观察组更低(P<0.05)。治疗后,两组CRP、TNF-α及IL-6均降低(P<0.05),且观察组更低(P<0.05)。治疗后,两组Scr、mALB及β2-MG均下降(P<0.05),且观察组更低(P<0.05)。两组总并发症发生率及复发率无显著差异(P>0.05)。结论 对输尿管结石患者实施消瘀化石汤联合体外冲击波碎石术排石效果显著,将降低患者中医证候积分及血清炎症因子水平,改善患者肾功能,且未增加不良反应发生率。Objective To investigate the effect of Xiaoyu Huashi Decoction(消瘀化石汤)combined with extracorporeal shock wave lithotripsy on stone excretion and serum inflammatory factors in patients with ureteral calculi.Methods The clinical data of 80 patients with ureteral calculi was retrospectively analyzed.Forty patients treated with extracorporeal shock wave lithotripsy alone were in the control group,and 40 patients treated with Xiaoyu Huashi Decoction+extracorporeal shock wave lithotripsy were in the observation group.The treatment lasted for 14 days and 6 months of follow-up was completed.The clinical efficacy,TCM symptom scores,serum inflammatory factors[C-reactive protein(CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)]and renal function[serum creatinine(Scr),microalbumin(mALB)andβ2-microglobulin(β2-MG)]before and after treatment were compared between the two groups.And the complications were recorded during treatment.After 6 months of follow-up,the recurrence of the two groups was compared.Results The total effective rate of the observation group was higher than that of the control group(P<0.05).After treatment,the scores of each dimension of TCM symptom scores in the two groups decreased(P<0.05),and those in the observation group were lower(P<0.05).After treatment,the levels of CRP,TNF-αand IL-6 in the two groups decreased(P<0.05),and those in the observation group were lower(P<0.05).After treatment,the levels of Scr,mALB andβ2-MG in both groups decreased(P<0.05),and those in the observation group were lower(P<0.05).There was no significant difference in the total complication rate or recurrence rate between the two groups(P>0.05).Conclusion Xiaoyu Huashi Decoction combined with extracorporeal shock wave lithotripsy has a significant effect on patients with ureteral calculi,which can reduce the scores of TCM symptoms and serum inflammatory factors and improve the renal function of patients,without increasing the incidence of adverse reactions.
