新生化颗粒联合芬吗通在稽留流产清宫术后促进子宫内膜修复效果及机制探讨

摘　　要：目的 观察新生化颗粒联合芬吗通在稽留流产清宫术后促进子宫内膜修复效果,并探讨其作用机制。方法 选取该院90例脾虚型慢性腹泻患者,分为常规组行参苓白术片常规药物治疗,研究组行矩阵针灸治疗,两组各45例,对比两组治疗后1个月的效果。结果 观察组术后腹痛消失时间、阴道流血时间、月经恢复时间短于对照组,阴道流血量少于对照组,子宫内膜厚度高于对照组,两组比较有显著性差异(P<0.05)。治疗前,两组中医证候积分比较,无显著性差异(P>0.05)。治疗后,两组阴道流血、色黯有块、面色青暗、小腹疼痛、少气懒言、口出恶臭积分均较前下降,且观察组低于对照组,有显著性差异(P<0.05)。治疗前,两组血清激素及缺氧诱导因子-1α(HIF-1α)、基质金属蛋白酶-9(MMP-9)比较,无显著性差异(P>0.05)。治疗后,两组雌二醇(E2)较前升高,孕酮(P)、黄体生成素(LH)、HIF-1α、MMP-9较前下降,而卵泡刺激素(FSH)与治疗前比较,无显著性差异(P>0.05)。观察组治疗后E2高于对照组,FSH与对照组比较无显著性差异(P>0.05),其他因子低于对照组,有显著性差异(P<0.05)。观察组月经复潮后整体经量优于对照组,而累计不良反应发生率与对照组比较,无显著性差异(P>0.05)。结论 新生化颗粒联合芬吗通用于稽留流产清宫术后可有效促进子宫内膜的修复,减少中医证候积分,改善月经复潮后经量,可能与调节血清性激素和HIF-1α、MMP-9有关。Objective To observe the effect of Xinshenghua Granules(新生化颗粒)combined with fenmorton in promoting endometrial repair after clearing uterine after missed abortion and to explore its mechanism.Methods A total of 100 patients with missed abortion who were admitted to the obstetrics and gynecology department of the hospital from January 2018 to December 2021 were divided into two groups by random number table method.Both groups were treated with clearing uterine therapy,50 cases in the control group were treated with fenmoton after operation,and the other 50 cases in the observation group were treated with Xinshenghua Granules combined with fenmoton after operation.The postoperative recovery,TCM syndrome scores,serum factors,overall menstrual volume and safety after menstruation were compared between the two groups.Results The postoperative abdominal pain disappearance time,vaginal bleeding time and menstrual recovery time in the observation group were shorter than those in the control group.The amount of vaginal bleeding was less than that in the control group,and the endometrial thickness was higher than that in the control group(P<0.05).Before treatment,there was no difference in TCM syndrome scores between the two groups(P>0.05).After treatment,the scores of vaginal bleeding,dark and lumpy color,dark complexion,abdominal pain,lack of breath,lazy speech and bad odor in the two groups were all decreased compared with those before treatment,and the indexes in the observation group were significantly lower than those of the control group(P<0.05).Before treatment,there was no difference in serum hormones,hypoxia-inducible factor-1α(HIF-1α)or matrix metalloproteinase-9(MMP-9)between the two groups(P>0.05).After treatment,the estradiol(E2)level in the two groups increased compared with that before treatment.The levels of progesterone(P),luteinizing hormone(LH),HIF-1αand MMP-9 decreased compared with those before treatment,and follicle stimulating hormone(FSH)was the same as that before treatment(P>0.05).Af
