针刀松解结合益气活血通络汤治疗早中期膝骨关节炎顽固性疼痛效果观察

摘　　要：目的 观察针刀松解结合益气活血通络汤治疗早中期膝骨关节炎顽固性疼痛效果。方法 将该院收治的101例早中期膝骨关节炎顽固性疼痛患者(2020年1月—2022年1月)纳入实验研究,以随机数字表法将患者分为治疗组(51例)与对照组(50例),对照组患者采取萘丁美酮胶囊治疗,治疗组患者采取萘丁美酮胶囊及针刀松解与益气活血通络汤治疗,各组数据观察:治疗效果、治疗前后疼痛程度(VAS)评分变化及膝关节功能量表(WOMAC)评分变化、膝关节活动度变化、不良反应等指标。结果 治疗组患者治疗总有效率比对照组患者更高,P<0.05;治疗前,各组患者VAS评分及WOMAC评分、膝关节活动度等指标比较,P>0,05,治疗后各组患者VAS评分及WOMAC评分、膝关节活动度等指标均改善,治疗组患者治疗后VAS评分及WOMAC评分、膝关节活动度等指标均优于对照组患者,P<0.05。两组患者不良反应发生率均较低,P>0.05。结论 针刀松解结合益气活血通络汤治疗早中期膝骨关节炎顽固性疼痛效果显著,患者疼痛显著改善,且膝关节功能恢复较好,不良反应较少,较为安全可靠。Objective To observe the therapeutic effect of acupotomology loosening and Yiqi Huoxue Tongluo Decoction(益气活血通络汤)on intractable pain of knee osteoarthritis in early and middle stages.Methods A total of 101 patients with intractable pain of knee osteoarthritis in the early and middle stages(from January 2020 to January 2022)admitted to the hospital were included in the experimental study.The patients were divided into a treatment group(51 cases)and a control group(50 cases)by random number table method.The control group was treated with nabumethone capsules.The patients in the treatment group were treated with nabumethone capsule,acupotomology loosening and Yiqi Huoxue Tongluo Decoction.The therapeutic effect,changes of pain degree(VAS)score before and after treatment,changes of knee function scale(WOMAC)score,changes of knee range of motion,adverse reactions and other indicators of two groups were compared.Results The total effective rate of the treatment group was higher than that of the control group(P<0.05).Before treatment,VAS score,WOMAC score,knee motion and other indicators of both groups were not significant(P>0.05).After treatment,VAS score,WOMAC score,knee motion and other indicators of both groups were improved.After treatment,VAS score,WOMAC score,knee motion and other indicators of the treatment group were better than those of the control group(P<0.05).The incidence of ADR was lower in two groups(P>0.05).Conclusion Acupotomology loosening combined with Yiqi Huoxue Tongluo Decoction has a significant effect on the treatment of intractable pain of knee osteoarthritis in the early and middle stages.The pain of patients has been significantly improved,and the functional recovery of knee joint is better,with fewer adverse reactions.It is relatively safe and reliable.
