安宁疗护在社区终末期疾病患者中的应用效果

摘　　要：目的:探讨安宁疗护在社区终末期疾病患者干预中的应用效果。方法:选取2015年1月—2018年12月北京市西城区德胜社区卫生服务中心收治的150例终末期疾病患者作为研究对象,随机分为两组,各75例。对照组给予常规干预,观察组开展安宁疗护。比较两组干预效果。结果:干预前,两组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分比较,差异无统计学意义(P>0.05);干预后,两组SAS、SDS评分低于干预前,且观察组低于对照组,差异有统计学意义(P<0.05)。干预前,两组数字分级法(NRS)、Barthel指数、简易营养评估量表(MNA)评分比较,差异无统计学意义(P>0.05);干预后,两组NRS评分低于干预前,且观察组低于对照组,两组Barthel指数、MNA评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。观察组总满意度高于对照组,差异有统计学意义(P<0.001)。结论:安宁疗护在社区终末期疾病患者中的应用效果较好,能够改善患者心理状态、营养状态,减轻疼痛,提高患者生活质量。Objective:To explore the application effect of palliative care in end-stage disease patients in the community.Methods:A total of 150 patients with end-stage disease who were admitted to Beijing Xicheng District Desheng Community Health Service Center from January 2015 to December 2018 were selected as the study subjects.They were randomly divided into two groups with 75 cases in each group.The control group was given routine care,and the observation group was given palliative care.The intervention effect was compared between the two groups.Results:Before intervention,there was no significant difference in the scores of Self-Rating Anxiety Scale(SAS)and Self-Rating Depression Scale(SDS)between the two groups(P>0.05).After intervention,the SAS and SDS scores in the two groups were lower than those before intervention,the scores in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Before intervention,there was no significant difference in Numeric Rating Scale(NRS)scores,Battle index,Mini-Nutritional Assessment Short-Form(MNA)scores between the two groups(P>0.05).After intervention,the NRS scores in the two groups were lower than those before intervention,and the NRS scores in the observation group were lower than those in the control group,while the Barthel index and MNA scores in the two groups were higher than those before intervention,and the Barthel index and MNA scores in the observation group were higher than those in the control group,and the difference was statistically significant(P<0.05).The total satisfaction in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.001).Conclusion:The application effect of palliative care end-stage disease patients in the community is good,which can improve the psychological state and nutritional status of patients,relieve pain and improve the quality of life of patients.
