摘　　要：目的:观察封闭负压引流技术联合刃厚皮片移植术治疗难治性溃疡的临床效果。方法:选取2020年6月—2022年12月广东省清远市中医院收治的难治性溃疡患者60例作为研究对象,根据随机数字表法分为两组,各30例。两组患者均给予常规创面清理以及抗感染治疗,对照组采用烧伤膏联合微粒皮移植术治疗,观察组采用封闭负压引流技术联合刃厚皮片移植术治疗。比较两组治疗效果。结果:两组创面肉芽组织生长情况比较,差异无统计学意义(P>0.05)。观察组肉芽组织生长达到植皮要求时间、创面愈合时间短于对照组,住院总费用少于对照组,差异有统计学意义(P<0.05)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。观察组预后总满意度高于对照组,差异有统计学意义(P=0.038)。结论:封闭负压引流技术联合刃厚皮片移植术治疗难治性溃疡的临床效果显著,能够促进肉芽组织生长,缩短创面愈合时间,减少住院总费用,患者满意度高。Objective:To observe the clinical effect of closed negative pressure drainage technology combined with bladethickness skin grafting in the treatment of refractory ulcer.Methods:From June 2020 to December 2022,60 patients with refractory ulcer who were admitted to Qingyuan Hospital of Traditional Chinese Medicine in Guangdong Province were selected as the study subjects.They were divided into two groups according to random number table method,with 30 cases in each group.The patients in both groups received conventional wound debridement and anti-infective therapy.The control group was treated with burn ointment combined with micro-skin grafting,and the observation group was treated with closed negative pressure drainage technology combined with blade-thickness skin grafting.The treatment effects were compared between the two groups.Results:There was no significant difference in the wound granulation tissue growth between the two groups(P>0.05).The time for wound granulation tissue growth to meet the requirements of skin grafting and the time for wound healing in the observation group were shorter than those in the control group,the total hospitalization expenses in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the total incidence of complications between the two groups(P>0.05).The total satisfaction with prognosis in the observation group was higher than that in the control group,and the difference was statistically significant(P=0.038).Conclusion:Closed negative pressure drainage technology combined with blade-thickness skin grafting has significant clinical effects in the treatment of refractory ulcer,and can promote the growth of granulation tissue,shorten the wound healing time,reduce the total hospitalization expenses,and achieve high patient satisfaction.
