超声引导下尿激酶局部溶栓联合经皮血管成形术治疗自体动静脉内瘘血栓形成的效果分析

摘　　要：目的:分析超声引导下尿激酶局部溶栓联合经皮血管成形术(PTA)治疗自体动静脉内瘘(AVF)血栓形成的效果。方法:选取2020年1月—2023年12月北京核工业医院收治的AVF血栓形成患者80例作为研究对象,随机分为对照组和观察组,各40例。对照组实施PTA治疗,观察组在对照组基础上实施超声引导下尿激酶局部溶栓治疗。比较两组肱动脉血流速度、生存质量、并发症发生情况。结果:治疗前,两组肱动脉血流速度比较,差异无统计学意义(P>0.05);治疗后,两组肱动脉血流速度高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.001)。治疗前,两组生理功能、生理职能、躯体疼痛、总体健康、活力、社会功能、情感职能、精神健康评分比较,差异无统计学意义(P>0.05);治疗后,两组生理功能、生理职能、躯体疼痛、总体健康、活力、社会功能、情感职能、精神健康评分均提高,且观察组高于对照组,差异有统计学意义(P<0.001)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:超声引导下尿激酶局部溶栓联合PTA治疗AVF血栓形成的效果显著,可提高肱动脉血流速度,改善患者生活质量,且安全性较高。Objective:To analyze the effect of ultrasound-guided local thrombolytic urokinase combined with percutaneous angioplasty(PTA)in the treatment of autogenous arteriovenous fistula(AVF)thrombosis.Methods:A total of eighty patients with AVF thrombosis admitted to Beijing Nuclear Industry Hospital from January 2020 to December 2023 were selected as the study objects and randomly divided into control group and observation group,with forty patients in each group.The control group was treated with PTA,and the observation group was treated with local thrombolytic therapy of urokinase guided by ultrasound.Brachial artery flow velocity,quality of life and complications were compared between the two groups.Results:Before treatment,there was no significant difference in brachial artery flow velocity between the two groups(P>0.05);after treatment,the blood flow velocity of brachial artery in the two groups was higher than before treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.001);before treatment,there were no significant differences in physiological function,physiological function,physical pain,general health,vitality,social function,emotional function and mental health scores between the two groups(P>0.05);after treatment,the scores of physiological function,physiological function,physical pain,general health,vitality,social function,emotional function and mental health in both groups were improved,and the observation group was higher than the control group,the difference was statistically significant(P<0.001);there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Ultrasound-guided local thrombolytic therapy of urokinase combined with PTA is effective in the treatment of AVF thrombosis,which can increase brachial artery blood flow velocity and improve patients'quality of life with high safety.
