门冬胰岛素30注射液联合利格列汀治疗对老年T2DM患者胰岛素抵抗、胰岛β细胞分泌的影响

摘　　要：目的分析老年T2DM患者接受门冬胰岛素30注射液联合利格列汀治疗的效果。方法研究对象为我院住院治疗的老年T2DM患者,选取时间2020年1月至2022年12月,选取患者78例,分组方法:随机数字表法,分为对照组、观察组,每组39例,治疗方法:对照组采用门冬胰岛素30注射液,观察组在对照组基础上采用利格列汀。比较两组患者血糖控制效果、胰岛β细胞分泌情况、胰岛素抵抗情况、治疗效果、不良反应。结果空腹血糖(FBG)、餐后2 h血糖含量(2 hPG)、糖化血红蛋白(HbAlc)指标比较,治疗前两组患者对比无差异(P>0.05),治疗后观察组低于对照组(P<0.05);胰岛β细胞功能指数(HOMA-β)比较,治疗1个月、3个月、6个月后,观察组高于对照组(P<0.05);胰岛素抵抗指数(HOMA-IR)比较,治疗1个月、3个月、6个月后,观察组低于对照组(P<0.05);治疗总有效率比较,观察组高于对照组(P<0.05);不良反应发生率比较,两组对比无差异(P>0.05)。结论门冬胰岛素30注射液联合利格列汀治疗,能改善T2DM老年患者胰岛素抵抗,优化血糖指标,安全性高。Objective To analyze the efficacy of NovoMix 30 FlexPen plus ligliptin in elderly patients with T2DM.Methods The subjects were hospitalized elderly T2DM patients,selected from January 2020 to December 2022,with 78 patients selected.The grouping method is a random number table,divided into a control group and an observation group,with 39 cases in each group.The treatment method is as follows:the control group uses insulin 30 injection,while the observation group uses lipagliptin on the basis of the control group.Compare the blood glucose control effect and pancreatic islets between two groups of patients β Cell secretion,insulin resistance,treatment efficacy,and adverse reactions.Results There was no difference in FBG,2 hPG and HbAlc before treatment between the two groups(P>0.05).After treatment,the levels of FBG,2 hPG and HbAlc in the observation group were lower than those in the control group(P<0.05);after 1,3 and 6 months of treatment,the level of beta-cell function index(HOMA-β)in the observation group was higher than that in the control group(P<0.05),Homa-ir in the observation group was lower than that in the control group after 1 month,3 months and 6 months treatment(P<0.05);the incidence of adverse reactions there was no difference between the two groups(P>0.05).Conclusions NovoMix 30 FlexPen and ligagliptin can improve insulin resistance,optimize blood glucose level and have high safety in elderly patients with T2DM.
