比较选择性痔上黏膜切除术与吻合器痔上黏膜环切术治疗混合痔的临床效果

摘　　要：目的探究在混合痔疾病中用选择性痔上黏膜切除术(TST)和吻合器黏膜环切术(PPH)分别治疗的临床效果。方法从本院肛肠科2022年7月至2023年7月期间接收的混合痔患者中选择100例为对象,分组方法为随机数字表法,对照组和观察组均有50例,分别用PPH、TST治疗。对两组围手术期情况、视觉模拟量表(VAS)评分、肛内坠胀感、并发症进行比较。结果观察组术中出血量少于对照组,住院时间均短于对照组,但手术时间长于对照组(P<0.05);观察组术后24 h、48 h、72 h的肛内坠胀感评分及不同恢复时期VAS评分均低于对照组(P<0.05);并发症总发生率相比,观察组比对照组低(P>0.05)。结论于混合痔疾病患者中采用TST治疗的效果更理想,可改善围手术期情况,减轻术后痛感,减轻肛内坠胀感,安全性较高。Objective To investigate the clinical effects of selective hemorrhoidal mucosal resection(TST)and stapler mucosal resection(PPH)in mixed hemorrhoidal diseases.Methods A total of 100 mixed hemorrhoids patients(from July 2022 to July 2023)were selected from the anorectal department of our hospital.The grouping method was random number table,and 50 patients in the control group and observation group were treated with PPH and TST respectively.Perioperative conditions,visual analogue scale(VAS)scores,anal distention and complications were compared between the two groups.Results The intraoperative blood loss and hospital stay in the observation group were shorter than those in the control group,but the operative time was longer than those in the control group(P<0.05).The scores of anal distension and VAS at 24h,48h and 72h in the observation group were lower than those in the control group at different recovery periods(P<0.05).The overall incidence of complications was lower in the observed group than in the control group(P>0.05).Conclusions In patients with mixed hemorrhoid disease,TST treatment has a better effect,which can improve perioperative conditions,reduce postoperative pain,reduce the feeling of anal swelling,and has high safety.
