0.75%罗哌卡因联合舒芬太尼在瘢痕子宫孕产妇剖宫产手术麻醉中的效果  

Effectiveness of 0.75% Ropivacaine Combined with Sufentanil in Anesthesia for Cesarean Section in Pregnant Women with Scarred Uterus

摘　　要：目的比较0.75%罗哌卡因复合舒芬太尼和单纯0.75%罗哌卡因在瘢痕子宫孕产妇剖宫产手术中的麻醉效果。方法选取2021年7月至2022年6月在我院行择期剖宫产的剖宫产后再次妊娠的孕产妇为研究对象,采用随机数字表法将60例孕产妇分为对照组和试验组,每组各30例。所有孕产妇均行腰硬联合阻滞麻醉,试验组予0.75%罗哌卡因1.6 ml+舒芬太尼5μg,对照组予0.75%罗哌卡因1.6 ml+生理盐水1 ml。记录两组产妇麻醉起效时间、麻醉维持时间、麻醉前后的收缩压及舒张压以及术中缝合腹膜时牵拉反应发生情况。结果麻醉前后,两组收缩压和舒张压对比均无差异(均P>0.05);相比于对照组,试验组麻醉起效时间更短,麻醉维持时间更长,牵拉反应发生率更低(均P<0.05)。结论剖宫产再孕产妇剖宫产手术时采用0.75%罗哌卡因复合5μg舒芬太尼行腰硬联合麻醉不仅起效快且镇痛时间较长、术中牵拉反应少,麻醉效果确切。Objective Vergleich der Anästhesie mit 0.75% Ropivacain in Kombination mit Sufentanil und 0.75%Ropivacain allein beim Kaiserschnitt bei schwangeren Frauen mit vernarbter Gebärmutter.Methods Mothers who were re-pregnant after cesarean section who underwent elective cesarean section in our hospital from July 2021 to June 2022 were selected for the study,and 60 cases of pregnant women were divided into the control group and the experimental group using the random number table method,with 30 cases in each group.All pregnant women underwent combined lumbar and rigid block anaesthesia,with 0.75% ropivacaine 1.6 ml+sufentanil 5μg in the experimental group and 0.75% ropivacaine 1.6 ml+saline 1 ml in the control group.the onset time of anaesthesia,duration of anaesthesia,pre-and post-anaesthesia systolic and diastolic blood pressure as well as the incidence of pulling reaction during intra-operative suturing of the peritoneum were recorded in the two groups.Results Before and after anaesthesia,there was no difference in the comparison of systolic and diastolic blood pressure between the two groups(all P>0.05);compared with the control group,the experimental group had a shorter onset of anaesthesia,longer anaesthesia maintenance,and a lower incidence of pulling reactions(all P<0.05).Conclusions 0.75% ropivacaine combined with 5μg sufentanil for combined spinal-epidural anesthesia in cesarean section for parturient women is not only quick onset of analgesia,but also has longer analgesic time,less intraoperative traction reaction,exact anesthesia effect.
