中药外敷方联合红外线照射对自体动静脉内瘘成熟的临床疗效分析

摘　　要：目的:探讨中药外敷方联合红外线(TDP)照射促进自体动静脉内瘘成熟的有效性、安全性。方法:随机选取50例行动静脉内瘘成形术患者,分为观察组和对照组,每组25例。对照组接受常规治疗及护理,观察组在此基础上采用中药外敷方联合TDP照射,观察2组术后当天、术后1月、术后2月患者血管头静脉内径、肱动脉血流量及距皮深度等指标的变化,比较2组内瘘成熟时间、并发症发生情况。结果:术后1、2月,观察组头静脉内径、肱动脉血流量均比对照组明显增大,距皮深度明显降低,统计学差异显著;观察组自体动静脉内瘘平均成熟所需时间短于对照组,术后1、2月的成熟比例高于对照组,差异有统计学意义;在整个试验观察周期中,观察组不良事件发生率低于对照组,差异有统计学意义。结论:中药外敷方配联合TDP照射能有效促进内瘘成熟、提高内瘘质量,降低并发症发生率。Objective:To explore the effectiveness and safety of traditional Chinese medicine external application combined with infrared(TDP)irradiation in promoting the maturation of autologous arteriovenous fistula.Methods:Fifty patients who underwent arteriovenous fistula formation were randomly divided into an observation group and a control group,with 25 cases in each group.The control group received conventional treatment and nursing,while the observation group received traditional Chinese medicine external application combined with TDP irradiation on this basis.The changes in the internal diameter of the cephalic vein,the blood flow of the brachial artery,and the depth from the skin of the patients in the two groups were observed on the day after surgery,1 month after surgery,and 2 months after surgery.The maturation time of fistula and the occurrence of complications were compared between the two groups.Results:1 and 2 months after surgery,the internal diameter of the cephalic vein and the blood flow of the brachial artery in the observation group were significantly larger than those in the control group,and the depth from the skin was significantly reduced,with significant statistical differences;the average maturation time of autologous arteriovenous fistula in the observation group was shorter than that in the control group,and the maturation ratio 1 and 2 months after surgery was higher than that in the control group,with statistically significant differences;during the entire experimental observation period,the incidence of adverse events in the observation group was lower than that in the control group,with statistically significant differences.Conclusion:Chinese medicine external application prescription combined with TDP irradiation can effectively promote the maturation of fistula,improve the quality of fistula,and reduce the incidence of complications.
