温阳疏风汤加减联合左西替利嗪治疗慢性荨麻疹的疗效及对血清MCP、PARC/CCL-18的影响

摘　　要：目的:观察温阳疏风汤加减联合左西替利嗪治疗慢性荨麻疹的疗效及对血清单核细胞趋化蛋白(MCP)、肺部活化调节趋化因子/趋化因子配体18(PARC/CCL-18)的影响。方法:选取我院于2020年10月~2023年5月收治的慢性荨麻疹患者130例,根据治疗方法分为两组,每组65例。对照组给予左西替利嗪治疗,观察组给予温阳疏风汤加减联合左西替利嗪治疗。记录两组皮肤病生活质量指数(DLQI)评分及Th1/Th2因子、MCP、PARC/CCL-18的变化,统计两组疗效及不良反应。结果:观察组总有效率为95.38%,高于对照组,有统计学意义(P<0.05)。治疗后,两组干扰素-γ(IFN-γ)较治疗前上升(P<0.05),单核细胞趋化蛋白~1(MCP-1)、单核细胞趋化蛋白~3(MCP-3)、PARC/CCL-18、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)、白细胞介素-10(IL-10)较治疗前下降(P<0.05),观察组IFN-γ高于对照组(P<0.05),MCP、PARC/CCL-1、IL-4、TNF-α、IL-10低于对照组(P<0.05)。治疗后,两组DLQI评分(包括症状和感觉、人际关系、休闲活动、日常活动、工作和学习和临床治疗)较治疗前下降(P<0.05),且观察组DLQI评分低于对照组(P<0.05)。两组不良反应比较,无统计学意义(P>0.05)。结论:温阳疏风汤加减联合左西替利嗪治疗慢性荨麻疹可提高疗效和生活质量,可能与调节Th1/Th2因子、MCP、PARC/CCL-18的表达有关。Objective To observe the efficacy of Wenyang Shufeng decoction combined with levocetirizine in the treatment of chronic urticaria and its effect on serum monocyte chemoattractant protein(MCP)and lung activation regulating chemokine/chemokine ligand 18(PARC/CCL-18)in the treatment of chronic urticaria.Methods 130patients with chronic urticaria admitted in our hospital in October 2020~May 2023were divided into two groups according to the treatment method,65cases in each group.The control group was treated with levoxetirizine,and the observation group was treated with Wenyang Shufeng decoction combined with levoxetirizine.The scores of quality of life index(DLQI)and the changes of Th1/Th2factors and MCP、PARC/CCL-18were recorded groups were recorded.Results A total effective rate of 95.38%in the observation group was higher than that in the control group(P<0.05).Two groups of interferon-γ(IFNγ)increased after treatment(P<0.05),Monocyte chemoattractant protein-1(MCP-1),monocyte chemoattractant protein-3(MCP-3),P ARC/CCL-18,interleukin-4(IL-4),tumor necrosis factor-α(TNF-α),interleukin-10(IL-10)decreased compared to pre-treatment(P<0.05),The IFN-γin the observation group was higher than that in the control group(P<0.05)and the MCP、PARC/CCL-1、IL-4、TNF-α、IL-10was lower than that in the control group(P<0.05).The DLQI scores of the two groups(including symptoms and feelings,interpersonal relationships,leisure activities,daily activities,work and study,and clinical treatment)decreased after treatment(P<0.05),and the DLQI scores of the observation group were lower than those of the control group(P<0.05).Compared with the two groups,there was no statistical significance(P>0.05).Conclusion Wenyang Shufeng decoction combined with levoxetirizine in the treatment of chronic urticaria can improve the efficacy and quality of life,which may be related to the expression of Th1/Th2factors and MCP、PARC/CCL-18.
