改良B-Lynch缝扎术与双侧子宫动脉结扎术治疗剖宫产产后出血患者的效果比较

摘　　要：目的:比较改良B-Lynch缝扎术与双侧子宫动脉结扎术治疗剖宫产产后出血患者的效果。方法:选取2020年2月至2023年2月该院收治的128例剖宫产产后出血患者进行前瞻性研究,按照随机数字表法将其分为观察组与对照组各64例。对照组行双侧子宫动脉结扎术治疗,观察组行改良B-Lynch缝扎术治疗,比较两组止血有效率、手术相关指标(手术时间、剖宫产术后24 h出血量、住院时间、子宫切除率)水平、凝血功能指标[血浆凝血因子Ⅷ、凝血因子Ⅸ、纤维蛋白原(FIB)]水平和并发症发生率。结果:观察组手术时间和住院时间均短于对照组,剖宫产术后24 h出血量少于对照组,差异均有统计学意义(P<0.05);术后3 d,两组血浆凝血因子Ⅷ、凝血因子Ⅸ、FIB等凝血功能指标水平均高于术前,且观察组高于对照组,差异均有统计学意义(P<0.05);两组止血有效率、子宫切除率和并发症发生率比较,差异均无统计学意义(P>0.05)。结论:改良B-Lynch缝扎术治疗剖宫产产后出血患者可提高凝血功能指标水平,缩短手术时间和住院时间,减少剖宫产术后24 h出血量,效果优于双侧子宫动脉结扎术治疗。Objective:To compare effects of modified B-Lynch suture and bilateral uterine artery ligation in treatment of patients with postpartum hemorrhage after cesarean section.Methods:A prospective study was conducted on 128 patients with postpartum hemorrhage after cesarean section admitted to the hospital from February 2020 to February 2023.According to the random number table method,they were divided into observation group and control group,64 cases in each group.The control group was treated with bilateral uterine artery ligation,while the observation group was treated with modified B-Lynch suture.The hemostatic efficiency,the operation-related indicators(operation time,24 h bleeding volume after cesarean section,hospitalization time,hysterectomy rate)levels,the coagulation function indicators[plasma coagulation factor Ⅷ,coagulation factor Ⅸ,fibrinogen(FIB)]levels,and the incidence of complications were compared between the two groups.Results:The operation time and the hospitalization time of the observation group were shorter than those of the control group,the 24 h bleeding volume after cesarean section was lower than that of the control group,and the differences were statistically significant(P<0.05).3 d day after the surgery,the levels of coagulation function indexes such as plasma coagulation factor VIII,coagulation factor IX and FIB in the two groups were higher than those before operation,those in the observation group were higher than those in the control group,the differences and were statistically significant(P<0.05).However,there were no significant differences in the hemostatic efficiency,the hysterectomy rate and the incidence of complications between the two groups(P>0.05).Conclusions:Modified B-Lynch suture in the treatment of the patients with postpartum hemorrhage after cesarean section can improve the levels of coagulation function indicators,shorten the operation time and the hospitalization time,and reduce the 24 h bleeding volume after cesarean section.Moreover,it is superior to bilateral ut
