摘　　要：目的探讨肝移植受者术前衰弱评估量表的编制并检验其用于肝移植术前患者的信效度,为有效开展肝移植受者术前衰弱管理提供方法学参考。方法根据文献研究、小组讨论初步拟定衰弱评估量表条目池;通过德尔菲法专家函询构建肝移植受者术前衰弱评估量表条目初稿;以便利抽样法选取30例肝移植术前患者进行小样本调查,检验量表的临床适用性;采用量表初稿对青岛大学附属医院2022年9月至2023年6月就诊的160例肝移植术前患者进行问卷调查,运用SPSS 25.0对数据进行统计分析,计算量表的信效度。结果肝移植受者术前衰弱评估量表包括26个条目,探索性因子分析提取5个公因子:疾病症状、生理状态、健康信息、社会参与、心理情绪,累积方差贡献率为62.03%。量表水平的内容效度指数为0.923,各条目水平的内容效度指数为0.857~1.000。量表内部一致性Cronbach sα系数、Spearman-Brown分半信度系数、重测信度分别为0.851、0.774、0.865。结论本研究编制的肝移植受者术前衰弱评估量表具有良好的信效度,对临床肝移植受者术前衰弱水平的评估,有一定应用价值。Objective To explore the development of a preoperative frailty assessment scale for liver transplant recipients and test its reliability and validity preoperatively,so as to provide a methodological reference for the effective management of preoperative frailty in liver transplant recipients.Methods Based on literature research and panel discussion,the scale item pool was preliminarily formulated.The first draft of the preoperative frailty assessment scale for liver transplant recipients was constructed through the Delphi method of expert consultation.A small sample of 30 preoperative liver transplantation patients was selected by convenience sampling method to test the clinical applicability of the scale.A total of 160 patients before liver transplantation in the Affiliated Hospital of Qingdao University from September 2022 to June 2023 were investigated with the first draft of the scale.SPSS 25.0 was used to analyze the data and calculate the reliability and validity of the scale.Results The preoperative frailty assessment scale for liver transplant recipients included 26 items,and exploratory factor analysis extracted 5 common factors which were disease symptoms,physiological status,health information,social participation,and psychology sentiments,with a cumulative variance contribution of 62.03%.The scale-content validity index was 0.923,and the item-content validity index was 0.857 to 1.000.Scale internal consistency Cronbach sαcoefficients,Spearman-Brown split half confidence coefficients,and retest reliability were 0.851,0.774,and 0.865,respectively.Conclusion The preoperative frailty assessment scale developed in this study demonstrates strong reliability and validity,and has a certain application value for evaluating preoperative frailty level of clinical liver transplant recipients.
