针灸治疗多囊卵巢综合征的临床疗效及作用机制研究进展

摘　　要：从中外数据库中检索有关中医外治法治疗多囊卵巢综合征PCOS的相关文献,经过总结中医外治在临床治疗PCOS的观察指标有:改善月经状况、降低雄激素水平及改善高雄激素的相关症状、改善子宫内膜容受性、提高排卵率、改变妊娠结局、降低体质量和改善机体对胰岛素的敏感性等。中医外治法包括:针灸、穴位埋线与耳穴等。本治疗组中医外治法可能的起效途径主要为:调控下丘脑-垂体-卵巢轴(HPO)、改善慢性炎症状态、调控PI3K/Akt/mTOR等通路与改善肠道菌群。中医外治法可能通过调节内分泌、缓解炎症和改善胰岛素敏感性等改善PCOS的临床症状。The relevant literature on the treatment of PCOS with TCM external therapies were retrieved from both domestic and foreign databases.After summarizing,the clinical observation indicators for TCM external therapies of PCOS include improvements in menstrual status,reduction in androgen levels,improvements of symptoms related to high androgen,improvement of endometrial receptivity,increase in ovulation rate,change of pregnancy outcome,reduction in body weight and improvement in insulin sensitivity.The TCM external therapies include acupuncture and moxibustion,acupoint catgut embedding,auricular needling.The potential effects of TCM external therapies mainly involve regulating hypothalamic-pituitary-ovarian axis,improving chronic inflammation,regulating PI3K/Akt/mTOR pathway,and improving intestinal flora.TCM external therapies may improve the clinical symptoms of PCOS by regulating endocrine,relieving inflammation and improving insulin sensitivity.
