摘 要:目的分析彝族和汉族人群中高血压、甘油三酯葡萄糖-体重指数(TyG-BMI)与代谢综合征(metabolic syndrome,MetS)患病的关联性,探讨TyG-BMI在高血压和MetS关系的中介效应。方法采用多阶段、分层整群抽样方法,抽取安宁市1051个彝族和19532个汉族作为研究对象。采用logistic回归模型分析高血压、TyG-BMI与MetS的关联性,计算MetS患病风险的OR值(95%CI)。利用中介效应模型分析TyG-BMI在高血压与MetS关系中的中介效应值。结果相比非高血压组,两个民族中高血压组BMI、TC、UA、TyG-BMI等指标水平都更高。回归分析结果显示,不管是彝族还是汉族,患高血压相对于不患的人发生MetS的风险增加,在进一步调整有关变量后,Model 2和Model 3中彝族患病风险分别为4.91(95%CI:3.17~7.60)和5.21(95%CI:3.19~8.53),高于汉族的3.77(95%CI:3.45~4.12)和3.84(95%CI:3.49~4.23)。TyG-BMI指数与MetS患病风险呈正相关。随着TyG-BMI指数增加,汉族MetS患病风险的增加幅度更大,例如同为Model 3中,彝族的Q4组OR值相比于Q1组增加了54倍,汉族Q4组相比于Q1组增加了67倍。中介分析发现,TyG-BMI部分介导了高血压对MetS的发生(中介百分比:彝族33.33%,汉族34.42%)。结论彝族和汉族高血压导致MetS发生的部分原因由TyG-BMI介导,这种中介作用在彝族和汉族中差异并不大。Objective To to investigate the relationship between hypertension,TyG-BMI,and metabolic syndrome(MetS)in the Yi and Han ethnic groups,and to explore the mediating role of TyG-BMI in this relationship.Methods A total of 1051 Yi and 19532 Han individuals from Anning were selected using multi-stage and stratified cluster sampling.Logistic regression models were employed to analyze the association between hypertension,TyG-BMI,and MetS,calculating odds ratios(ORs)with 95%confidence intervals(CIs)for MetS risk.Mediation analysis was conducted to assess the mediating effect of TyG-BMI on the hypertension-MetS relationship.Results Compared to the non-hypertensive group,individuals with hypertension exhibited higher levels of BMI,TC,UA,and TyG-BMI.Regression analysis revealed that hypertension increased the risk of MetS compared to non-hypertensive individuals.After adjusting for relevant variables,the adjusted ORs(95%CI)for MetS risk in Yi individuals were 4.91(3.17-7.60)and 5.21(3.19-8.53)in Model 2 and Model 3,respectively,higher than those in Han individuals,which were 3.77(3.45-4.12)and 3.84(3.49-4.23).TyG-BMI was positively correlated with MetS risk,with increasing TyG-BMI associated with a significant increase in MetS risk among the Han population.For instance,in Model 3,the OR for the highest TyG-BMI quartile was 54 times higher in Yi individuals and 67 times higher in Han individuals compared to the lowest quartile.Mediation analysis indicated that TyG-BMI partially mediated the relationship between hypertension and MetS,with mediation percentages of 33.33%in Yi and 34.42%in Han individuals.Conclusion TyG-BMI partially mediates the association between hypertension and MetS in both Yi and Han ethnic groups,with no significant difference in this mediation observed between the two groups.
