摘 要:目的 探索上海市浦东新区小学女生血脂水平与青春期发育的关系,为青春期血脂异常的研究提供依据。 方法 采用分层整群随机抽样方法,于2020年在浦东新区抽取一年级女生作为研究对象,进行问卷调查和体格检查,并抽取静脉血检测胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)。2年后对该人群进行随访调查和体格检查,并抽取空腹静脉血,检测项目同基线。采用多重线性回归模型分析血脂水平和不同青春发育分期的关系。 结果 共纳入符合要求女生932人,其中6~7岁血脂水平TC、TG、HDL-C、LDL-C分别为4.15、1.02、1.52和2.17mmol/L。2年后随访时(8~9岁)四项指标分别为 4.57、0.76、1.66和2.64mmol/L。基线时受检对象血脂异常检出率19.96%,随访时为19.42%。基线和随访调查均显示,女生TG和HDL-C水平在不同青春期发育分期之间的差异均有统计学意义(均P<0.05)。结论 上海市浦东新区女生血脂异常检出率较高,应引起高度重视。青春期女生血脂水平在不同青春期发育会有不同,在评估青春期女生血脂水平时应考虑不同青春发育分期的影响。Objective To investigate the association between puberty development and four blood lipid components,providing insights into adolescent dyslipidemia.Methods Using stratified cluster random sampling,932 first-grade female students from Pudong New Area were selected in 2020.Baseline data were collected through physical examinations and questionnaires,with measurements of total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)levels.Follow-up surveys and physical examinations were conducted after 2 years with identical blood tests.A multiple linear regression model was employed to analyze the relationship between blood lipid levels and different stages of puberty.Results In girls aged 6 to 7 years in Pudong New Area,TC,TG,HDL-C,and LDL-C levels were 4.15 mmol/L,1.02 mmol/L,1.52 mmol/L,and 2.17 mmol/L,respectively.At the 2-year follow-up(8 to 9 years old),these levels were 4.57 mmol/L,0.76 mmol/L,1.66 mmol/L,and 2.64 mmol/L,respectively.The prevalence of dyslipidemia was 19.96%at ages 6 to 7 and 19.42%at ages 8 to 9.Both surveys indicated significant differences in TG and HDL-C levels among girls at different stages of puberty(P<0.05).Conclusion The high prevalence of dyslipidemia among female students in Pudong New Area warrants significant attention.Blood lipid levels are associated with indices of puberty development in adolescents.Evaluation of blood lipid levels should consider variations by puberty development stage.
