摘 要:目的 了解病毒性脑炎患者血清单核细胞趋化蛋白-1(MCP-1)、巨噬细胞移动抑制因子(MIF)与脑电图特征的相关性。方法 选取2020年1月-2023年6月本院收治的病毒性脑炎患者80例为患病组,另选同期进行体检的的健康志愿者80例为对照组,收集分析一般临床资料,ELISA检测血清MCP-1、MIF水平,Pearson法分析患者血清MCP-1、MIF与脑电图特征参数的相关性,多因素Logistic回归分析病毒性脑炎发生的影响因素,绘制受试者工作特征(ROC)曲线分析血清MCP-1、MIF对病毒性脑炎发生的诊断价值。结果 患病组与对照组的年龄、性别、体质量指数、吸烟史、饮酒史以及是否患高血压、高血脂之间差异无统计学意义(P>0.05);与对照组相比,患病组的血清TNF-α [(10.25±1.36)μg/L vs.(3.58±0.38)μg/L]、IL-6 [(101.05±11.35)ng/L vs.(39.52±4.32)ng/L]、MCP-1 [(84.65±8.58)pg/mL vs.(76.61±7.95)pg/mL]、MIF水平[(5.03±0.59)ng/mL vs.(4.45±0.48)ng/mL]以及α波指数[(26.51±2.72)vs.(25.31±2.65)]、β波指数[(15.62±1.84)vs.(11.20±1.35)]、δ波指数[(15.82±1.62)vs.(13.25±1.51)]、θ波指数[(25.62±2.67)vs.(22.43±2.35)]均显著升高(P<0.05);血清MCP-1、MIF水平与脑电波特征参数均呈正相关(P<0.05);血清MCP-1、MIF、TNF-α水平均为影响病毒性脑炎的发生的影响因素(P<0.05);血清MCP-1、MIF以及两者联合诊断的AUC分别为0.790、0.782、0.861,联合诊断显著优于MCP-1(Z=2.758,P=0.006)、MIF(Z=2.819,P=0.005)单独诊断。结论 在病毒性脑炎患者中,血清MCP-1、MIF水平升高,与脑电图特征呈正相关,且对病毒性脑炎发生具有一定辅助诊断价值。Objective To analyze the correlation between serum monocyte chemotactic protein-1(MCP-1),macrophage migration inhibitory factor(MIF) with electroencephalogram(EEG) characteristics in patients with viral encephalitis. Methods Eighty patients with viral encephalitis admitted to our hospital from January 2020 to June 2023 were regarded as the disease group, and another 80 healthy volunteers who underwent physical examinations were regarded as the control group. General clinical data were collected and analyzed. ELISA was applied to detect levels of serum MCP-1 and MIF. Pearson method was applied to analyze the correlation between serum MCP-1,MIF with EEG characteristic parameters in patients. Multivariate logistic regression was applied to analyze the influencing factors of viral encephalitis. Receiver operating characteristic(ROC) curve was plotted to analyze the diagnostic value of serum MCP-1 and MIF for the occurrence of viral encephalitis. Results There was no obvious statistical difference in the age, gender, body mass index, smoking history, alcohol consumption history, and the presences of hypertension and hyperlipidemia between the disease group and the control group(P>0.05). Compared with the control group, the levels of serum TNF-α [(10.25±1.36) μg/L vs.(3.58±0.38) μg/L],IL-6 [(101.05±11.35) ng/L vs.(39.52±4.32) ng/L],MCP-1 [(84.65±8.58) pg/mL vs.(76.61±7.95) pg/mL],MIF [(5.03±0.59) ng/mL vs.(4.45±0.48) ng/mL],and the α wave index [(26.51±2.72) vs.(25.31±2.65)],β wave index [(15.62±1.84) vs.(11.20±1.35)],δ wave index [(15.82±1.62) vs.(13.25±1.51)],θ wave index [(25.62±2.67) vs.(22.43±2.35)] in disease group were obviously higher(P<0.05). The levels of serum MCP-1 and MIF were positively correlated with the EEG characteristic parameters(P<0.05). The levels of serum MCP-1,MIF,and TNF-α were all influencing factors for the occurrence of viral encephalitis(P<0.05).
