Masquelet技术联合游离髂腹股沟皮瓣治疗复杂手外伤复合组织缺损的效果研究

摘　　要：目的:分析复杂手外伤复合组织缺损患者应用Masquelet技术联合游离髂腹股沟皮瓣治疗的临床效果。方法:选取2020年6月—2022年6月南宁市第二人民医院创伤手外科收治的复杂手外伤患者80例作为研究对象,随机分为对照组和观察组,各40例。对照组实施游离髂腹股沟皮瓣移植术,观察组实施Masquelet技术联合游离髂腹股沟皮瓣移植术。比较两组感染发生率、皮瓣成活总有效率、住院时间、疼痛程度。结果:观察组感染发生率低于对照组,差异有统计学意义(P<0.001)。观察组皮瓣成活总有效率高于对照组,差异有统计学意义(P=0.003)。观察组住院时间短于对照组,差异有统计学意义(P<0.001)。治疗前,两组视觉模拟评分法(VAS)评分比较,差异无统计学意义(P>0.05);治疗后,两组VAS评分降低,观察组低于对照组,差异有统计学意义(P<0.05)。结论:Masquelet技术联合游离髂腹股沟皮瓣治疗复杂手外伤复合组织缺损的应用效果良好,可降低术后感染率,提高皮瓣成活率,减轻术后疼痛程度,缩短住院时间。Objective:To analyze the clinical effect of Masquelet technique combined with free ilioinguinal flap in the treatment of complex hand injury complicated with tissue defect.Methods:A total of eighty patients with complex hand trauma admitted to the Department of Traumatic Hand Surgery,Nanning Second People's Hospital from June 2020 to June 2022 were selected as the study objects and randomly divided into control group and observation group,with forty cases in each group.The control group un-derwent free ilioinguinal skin flap transplantation,and the observation group underwent Masquelet combined with free ilioinguinal skin flap transplantation.The incidence of infection,total effective rate of skin flap survival,length of hospital stay and degree of pain were compared between the two groups.Results:The incidence of infection in the observation group was lower than that in the control group,the difference was statistically significant(P<0.001).The total effective rate of skin flap survival in the observation group was higher than that in the control group,the difference was statistically significant(P=0.003).The duration of hospitaliza-tion in the observation group was shorter than that in the control group,and the difference was statistically significant(P<0.001).Before treatment,there was no significant difference in visual analogue scale(VAS)scores between the two groups(P>0.05);after treatment,the VAS score of the two groups decreased,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).Conclusion:Masquelet technique combined with free ilioinguinal flap has a good effect in the treatment of complex hand injury complicated with tissue defect,which can reduce the postoperative infection rate,improve the survival rate of the flap,reduce the postoperative pain,and shorten the hospital stay.
