小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折手术中的应用价值

摘　　要：目的:分析小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折手术中的应用价值。方法:选取2021年12月—2022年12月于陕西省森工医院行下肢骨折手术的老年患者120例作为研究对象,随机分为试验组与对照组,各60例。对照组实施连续硬膜外麻醉,试验组开展小剂量轻比重腰麻-硬膜外联合麻醉。比较两组麻醉干预效果。结果:试验组手术、苏醒时间短于对照组,警觉/镇静评分优于对照组,差异有统计学意义(P<0.001);试验组麻醉起效、阻滞完善时间短于对照组,差异有统计学意义(P<0.001);麻醉后,两组心率、呼吸频率、平均动脉压降低,试验组心率、呼吸频率、平均动脉压高于对照组,差异有统计学意义(P<0.05);试验组不良反应发生率低于对照组,差异有统计学意义(P=0.047)。结论:小剂量轻比重腰麻-硬膜外联合麻醉在老年下肢骨折手术中的麻醉效果更好,可缩短麻醉起效、阻滞完善时间,维持血流动力学稳定,安全性较高。Objective:To analyze the application value of low dose and light weight lumbar anesthesia combined with epidural anesthesia in elderly patients with lower extremity fracture surgery.Methods:A total of one hundred and twenty elderly patients who underwent lower limb fracture surgery in Shaanxi Sengong Hospital from December 2021 to December 2022 were selected as the study objects and randomly divided into experimental group and control group,with sixty cases in each group.The control group received continuous epidural anesthesia,and the experimental group received low dose light weight lumbar anesthesia combined epidural anesthesia.The effect of anesthesia intervention was compared between the two groups.Results:The operation and recovery time of experimental group was shorter than that of control group,and the alertness/sedation score was better than that of control group,the difference was statistically significant(P<0.001);the effect of anesthesia and the time of block completion in experimental group were shorter than those in control group,the difference was statistically significant(P<0.001);after anesthesia,the heart rate,respiratory rate and mean arterial pressure of the two groups were decreased,and the heart rate,respiratory rate and mean arterial pressure of the experimental group were higher than those of the control group,the difference was statistically significant(P<0.05);the incidence of adverse reactions in experimental group was lower than that in control group,and the difference was statistically significant(P=0.047).Conclusion:Low dose and light weight combined anesthesia of lumbar anesthesia and epidural anesthesia has better anesthetic effect in the operation of lower limb fracture in the elderly,which can shorten the effect of anesthesia,block perfect time,maintain hemodynamic stability and high safety.
