摘 要:目的:研究无创呼吸机联合氧气驱动雾化吸入治疗慢性阻塞性肺疾病(COPD)合并呼吸衰竭的效果。方法:选取2022年11月—2023年11月恩平市人民医院收治的COPD合并呼吸衰竭患者80例作为研究对象,随机分为对照组和观察组,各40例。对照组给予无创呼吸机治疗,观察组在对照组基础上联合氧气驱动雾化吸入治疗。比较两组血气指标、肺功能指标、并发症发生率。结果:治疗后,两组动脉血氧分压、动脉血氧饱和度水平高于治疗前,动脉血二氧化碳分压、呼吸频率水平低于治疗前,观察组优于对照组,差异有统计学意义(P<0.05);治疗后,两组第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)、FEV_(1)/FVC水平高于治疗前,气道压力水平低于治疗前,观察组优于对照组,差异有统计学意义(P<0.05);观察组并发症发生率低于对照组,差异有统计学意义(P=0.0449)。结论:无创呼吸机联合氧气驱动雾化吸入疗法治疗COPD合并呼吸衰竭的效果较好,可有效改善患者的肺功能、血气分析指标,降低并发症发生率。Objective:To study the effect of non-invasive ventilator combined with oxygen-driven nebulized inhalation in the treatment of chronic obstructive pulmonary disease(COPD)combined with respiratory failure.Methods:Eighty patients with COPD combined with respiratory failure admitted to Enping People's Hospital from November 2022 to November 2023 were se-lected as the study subjects,and randomly divided into the control group and the observation group,each with forty cases.The con-trol group was given non-invasive ventilator treatment,and the observation group was combined with oxygen-driven nebulized in-halation treatment on the basis of the control group.Comparison of blood gas indexes,lung function indexes,and complication rates between the two groups.Results:After treatment,the levels of arterial blood oxygen partial pressure and arterial blood oxy-gen saturation were higher than those before treatment,and the levels of arterial blood carbon dioxide partial pressure and respira-tory rate were lower than those before treatment,and the observation group was better than the control group,and the difference was statistically significant(P<0.05);after treatment,the levels of exertion expiratory volume in the first second(FEV_(1)),exertion lung capacity(FVC)and FEV_(1)/FVC of the two groups were higher than those before treatment,and the levels of airway pressure were lower than those before treatment,and the observation group was superior to the control group,and the difference was statisti-cally significant(P<0.05);the complication rate of the observation group was lower than that of the control group,and the differ-ence was statistically significant(P=0.0449).Conclusion:Non-invasive ventilator combined with oxygen-driven nebulized inhala-tion therapy is more effective in the treatment of COPD combined respiratory failure,which can effectively improve the lung func-tion and blood gas analysis indexes of the patients and reduce the complication rate.
