摘　　要：目的建立健康成人外周血T细胞亚群PD-1和CD28表达参考区间,并探究其与常规实验室指标的相关性。方法收集2022年11月至2023年1月在本院体检的120例健康体检者外周血,用流式细胞术检测淋巴细胞中CD3^(+)、CD4^(+)和CD8^(+)T细胞及其表面PD-1和CD28的表达,CD8^(+)T根据CD28和PD-1表达不同细分6个亚群,对这些指标的比例和绝对值建立参考范围。T细胞各亚群与常规指标进行相关性分析。结果健康成人外周血PD-1(%)表达率:CD4>CD3;CD28(%):CD4>CD3>CD8;PD1和CD28阳性计数:CD3>CD4>CD8。女性CD4^(+)(%)显著低于男性;年龄与CD8和CD3计数以及CD28计数负相关,与CD8^(+)CD28^(+)上PD-1表达呈正相关;红细胞以及血小板计数与CD8相关亚群上PD-1hi和PD-1int的表达和计数呈正相关,但和CD4^(+)(%)呈负相关。CD8^(+)CD28-亚群PD-1的表达和计数与ALB呈正相关;CD8^(+)CD28^(+)亚群PD-1计数和LDL呈正相关。结论本文建立了健康成人外周血T细胞亚群PD-1和CD28表达参考区间,并发现性别、年龄、红细胞、血小板、血脂和营养状态等对T细胞亚群以及PD-1和CD28表达均可造成影响。Objective To establish reference intervals for the expression PD-1 and CD28 in peripheral blood circulating T cells of healthy adults,and to explore their correlation with routine laboratory indicators.Methods Peripheral blood was collected from April 2023 to June 2023 from 120 healthy medical examiners in Zhejiang Cancer Hospital.CD3,CD4 and CD8 lymphocyte subsets and their expression of PD-1 and CD28 were analyzed by flow cytometry.CD8^(+)T cells were subdivided into six subgroups according to the differential expression levels of CD28 and PD-1.The reference ranges were established for the ratio and absolute values of these indicators.Correlation analysis was performed between subgroups of T cells and routine indicators.Results PD-1 expression rate was higher in CD4 T than CD3 T.CD28 expression rate was ranked as CD4>CD3>CD8.Absolute counts of PD1 and CD28 positive expression were ranked as CD3>CD4>CD8.Percent of CD4 T cell was significantly higher in females than males.Age was negatively correlated with CD8,CD3 and CD28 counts,and positively correlated with the PD-1 expression on CD8^(+)CD28^(+)subset.The expression and counts of PD-1hi and PD-1int on CD8 subpopulations were found to be positively correlated with both erythrocyte and platelet counts.However,they were negatively correlated with the percentage of CD4^(+)cells.Additionally,the expression and counts of CD8^(+)CD28-PD-1 subpopulations were positively correlated with albumin(ALB)levels,while count of CD8^(+)CD28^(+)PD-1 subpopulations were positively correlated with low-density lipoprotein(LDL)levels.Conclusion This article establishes a reference intervals for PD-1 and CD28 expression in adult peripheral blood T cell subsets and found that gender,age,erythrocytes,platelets,lipids and nutritional status can have an impact on T-cell subsets as well as PD-1 and CD28 expression.
