摘 要:目的探讨血气分析检测、血浆D-二聚体(D-D)和C反应蛋白(CRP)在血栓性肺栓塞分型中的应用效果。方法选取2021年9月至2022年7月该院74例血栓性肺栓塞患者,以栓塞范围为依据分为甲组(非大面积栓塞,37例)与乙组(大面积栓塞,37例),并选择同期37例健康体检者为对照组,检测3组受检者血气指标、D-D和CRP,分析检测结果。结果甲组及乙组氧分压比对照组低,而CRP及D-D较对照组高,差异有统计学意义(P<0.05);甲组二氧化碳分压比乙组及对照组低,差异有统计学意义(P<0.05);而乙组与对照组二氧化碳分压差异无统计学意义(P>0.05)。甲组氧分压阳性检出率比乙组低,而二氧化碳分压、D-D阳性检出率比乙组高,差异有统计学意义(P<0.05);甲组与乙组CRP阳性检出率差异无统计学意义(P>0.05)。结论血浆D-D、血气分析检测及CRP在血栓性肺栓塞分型诊断中具有较高的应用价值,可为疾病诊断及治疗提供参考,值得临床采纳、推广。Objective To explore the application effect of blood gas analysis detection,plasma D-dimer(DD)and C-reactive protein(CRP)in the classification of thrombotic pulmonary embolism.Methods Seventy four patients with thrombotic pulmonary embolism in Shanghai Integrated Traditional Chinese and Western Medicine Hospital from September 2021 to July 2022 were selected.They were divided into group A(37 cases of non massive embolism)and group B(37 cases of massive embolism)based on the extent of embolism.37 healthy individuals in the same period were selected as the control group,and blood gas indicators,D-D,and CRP were measured in the three groups.The test results were analyzed.Results The oxygen partial pressure in group A and group B was lower than that in the control group,while CRP and D-D were higher than those in the control group,and the differences were statistically significant(P<0.05);The partial pressure of car-bon dioxide in Group A was lower than that in Group B and the control group(P<0.05)and the difference was statistically significant;However,there was no significant difference in carbon dioxide partial pressure be-tween group B and the control group(P>0.05).The positive detection rate of oxygen partial pressure in group A was lower than that in group B,while the positive detection rates of carbon dioxide partial pressure and D-D were higher than those in group B,and the differences were statistically significant(P<0.05);There was no significant difference in the positive detection rate of CRP between Group A and Group B(P>0.05).Conclusion Plasma D-D,blood gas analysis,and CRP have high application value in the classification diagno-sis of thrombotic pulmonary embolism,and can provide reference for disease diagnosis and treatment,which is worthy of clinical adoption and promotion.
