摘 要:《要命还是要灵魂》中美国苗族女孩黎亚的就医案例是其求医的美国加利福尼亚州默塞德慈善医疗中心发生的有史以来最严重的医患冲突,是全球化背景下跨文化医疗矛盾的缩影。如何化解跨文化医疗中的医患冲突,促进全球范围内的公共卫生合作,是构建人类卫生健康共同体的重要议题。归本溯源,这场冲突包含了科学与人文、东方与西方、医生与患者的多元矛盾主体。从案例中的三对矛盾出发,解决跨文化的医患冲突,要从医生、社会、医院层面形成合力。医生要提升文化敏感性,培养跨文化能力,学会识别与尊重患者文化;在全社会范围内营造文化平等环境,促进跨文化医疗合作;医院需要健全体制,完善配套,不断创新与优化服务模式。The clinical encounter story of Li Ya,a Hmong American girl in The Spirit Catches You and You Fall Down,represents one of the most serious doctor-patient conflicts at the Mercy Merced Medical Center in California.It is a microcosm of cross-cultural medical conflicts in the era of globalization.Addressing such conflicts is crucial for promoting global public health cooperation and building a community of common health for mankind.The source of the conflicts is traced back to multiple contradictions between science and humanity,East and West,doctors and patients.Thus doctors,hospital and the whole society should work together to solve doctor-patient conflicts in cross-cultural clinical encounter.Doctors should enhance cultural sensitivity,cultivate cross-cultural competence,learn to recognize and respect patients'culture.Society should foster an equal cultural environment and promote cross-cultural medical cooperation.Hospitals need to improve systems,develop supportive measures,and continuously innovate and optimize service models.
