筋骨针针刺肩三针与手三针治疗肩周炎的效果及对疼痛程度、肩关节活动度的影响

摘　　要：目的:探讨筋骨针针刺肩三针与手三针治疗肩周炎的效果及对疼痛程度、肩关节活动度的影响。方法:将2019年5月—2023年5月收治的120例肩周炎患者随机分为对照组与三针组,各60例,对照组实施常规针刺疗法,三针组实施肩三针+手三针针刺疗法。比较两组临床疗效,治疗前后中医症状积分、肩关节疼痛度、活动度情况及血清生化指标[白细胞介素-6(IL-6)、转化生长因子-β(TGF-β)和前列腺素E2(PGE 2)]变化。结果:三针组临床总有效率为96.67%(58/60),高于对照组的86.67%(52/60),差异具有统计学意义(P<0.05);治疗后,两组中医症状积分、IL-6、TGF-β和PGE2水平均低于治疗前,肩关节活动角度高于治疗前,且三针组与对照组比较,差异具有统计学意义(P<0.05);治疗第1天、治疗第3天和治疗第1周两组肩关节疼痛度评分逐渐降低,差异具有统计学意义(P<0.05),且三针组显著低于对照组,差异具有统计学意义(P<0.05)。结论:筋骨针针刺肩三针与手三针治疗肩周炎效果显著,可有效减轻肩关节疼痛,提高活动度,改善临床症状与炎症反应,值得应用。Objective:To explore the effect of needling shoulder three points and hand three points with Jin Gu needle in treatment of scapulohumeral periarthritis and to observe its impact on pain severity and shoulder range of motion.Methods:120 patients with scapulohumeral periarthritis were randomly divided into the control group(n=60)and the study group(n=60).The control group was treated with conventional acupuncture,whereas the study group was treated with needling shoulder three points and hand three points with Jin Gu needle.Clinical efficacy,TCM symptoms scores,shoulder pain severity and range of motion,and serum biochemical indicators,including interleukin-6(IL-6),transforming growth factor-β(TGF-β)and prostaglandin E2(PGE 2),were compared between before and after the treatment in the two groups.Results:The total effective rate was 96.67%(58/60)in the study group,which was significantly higher than 86.67%(52/60)in the control group(P<0.05).The scores of TCM symptoms and the levels of IL-6,TGF-βand PGE 2 were lower after the treatment than those before the treatment in the two groups,and the shoulder range of motion was improved after the treatment than that before the treatment in the two groups;of which the improvements were more significant in the study group than those in the control group(P<0.05).The shoulder pain scores were gradually decreased on 1 st day,3 rd day and 1 st week of treatment in the two groups(P<0.05),and the scores were significantly lower in the study group than those in the control group(P<0.05).Conclusion:Needling shoulder three points and hand three points with Jin Gu needle has a significant effect on scapulohumeral periarthritis,and it can effectively relieve shoulder pain,improve shoulder range of motion,and improve clinical symptoms and inflammatory response.
