摘  要:目的:观察中药外用制剂甘草油治疗糖尿病足植皮术后伤口的临床效果。方法:将60例糖尿病足植皮术后患者按照随机数字表法分为两组,对照组30例植皮区伤口给予莫匹罗星纱条换药,治疗组30例植皮区伤口给予甘草油外用换药。观察两组疗效及术后第3天植皮成活率、术后第7天创面愈合率、术后前3次换药时疼痛情况。分析手术前1 d,手术后第3天、第7天患者创面渗液中基质金属蛋白酶-2(MMP-2)及MMP抑制剂-1(TIMP-1)含量变化。结果:治疗组术后第3天植皮成活率和术后第7天的创面愈合率分别为85.0%和90.0%,均高于对照组的60.0%和52.5%,差异有统计学意义(P<0.05);治疗组术后前3次换药时疼痛评分均低于对照组(P<0.05);治疗组手术后第3天创面渗液中MMP2质量浓度为(15.87±5.22)ng·mL^(-1),手术后第7天创面渗液中MMP-2质量浓度为(9.36±1.97)ng·mL^(-1),各指标均低于对照组(P<0.05);治疗组手术后第3天创面渗液中TIMP-1质量浓度为(26.36±4.16)ng·mL^(-1),手术后第7天创面渗液中TIMP-1质量浓度为(35.37±5.81)ng·mL^(-1),均高于对照组(P<0.05)。结论:甘草油外用治疗糖尿病足植皮术后伤口效果较好,能改善患者伤口炎症因子表达,促进伤口修复。Objective:To observe the clinical effect of traditional Chinese medicine(TCM)external preparation licorice oil in the treatment of wounds after diabetic foot skin grafting.Methods:Sixty patients with diabetic foot wounds after skin grafting were randomly divided into two groups according to a random number table.In the control group,30 patients were treated with mupirocin gauze for wound dressing,while in the treatment group,30 patients were treated with topical licorice oil for wound dressing.The efficacy,graft survival rate on the rd 3 postoperative day,wound healing rate on the th 7 postoperative day,and pain during the first three dressing changes were observed in both groups.Additionally,changes in the levels of matrix metalloproteinase-2(MMP-2)and matrix metalloproteinase inhibitor-1(TIMP-1),in wound exudate were analyzed on the day before surgery,the rd 3 postoperative day,and the th 7 postoperative day.Results:The treatment group showed a graft survival rate of 85.0%on the 3rd postoperative day and a wound healing rate of 90.0%on the th 7 postoperative day,both significantly higher than 60.0%and 52.5%in the control group(P<0.05).Pain scores during the first three dressing changes were significantly lower in the treatment group than in the control group(P<0.05).On the rd 3 postoperative day,the MMP-2 concentration in wound exudate in the treatment group was(15.87±5.22)ng·mL^(-1),and on the 7th postoperative day,it was(9.36±1.97)ng·mL^(-1),both significantly lower than those in the control group(P<0.05).The TIMP-1 levels in wound exudate in the treatment group were(26.36±4.16)ng·mL^(-1) on the rd 3 postoperative day and(35.37±5.81)ng·mL^(-1) on the th 7 postoperative day,both significantly higher than those in the control group(P<0.05).Conclusion:External application of licorice oil is effective in treating wounds after skin grafting in diabetic foot patients,which can improve the expression of wound inflammation factors and promote wound healing.
