中药熏蒸加氯诺昔康治疗腰椎间盘突出症临床观察

摘　　要：目的探讨腰椎间盘突出症(LDH)患者应用中药熏蒸加氯诺昔康治疗的效果。方法采用随机数字表法将2019年1月—2021年12月南丰县中医院收治的124例LDH患者分为两组,各62例。对照组给予氯诺昔康治疗,观察组加用中药熏蒸治疗,均连续治疗10 d。比较两组临床疗效、腰椎功能、疼痛程度、日常生活能力、血清学指标及不良反应。结果观察组临床总有效率为96.77%(60/62),高于对照组的85.48%(53/62)(P<0.05)。治疗后,观察组日本骨科协会评估治疗分数(JOA评分)、日常生活活动能力(ADL)评分较对照组高,视觉模拟量表(VAS)评分及肿瘤坏死因子-α(TNF-α)、重组人基质金属蛋白酶-3(MMP-3)水平较对照组低(P<0.05)。两组未见严重不良反应。结论LDH患者应用中药熏蒸加氯诺昔康治疗可减轻炎症反应,改善腰椎功能及日常生活活动能力,降低TNF-α、MMP-3水平,效果显著,安全性好,值得临床推广。Objective To investigate the effect of Chinese medicine fumigation and lornoxicam on patients with lumbar disc herniation(LDH).Methods A total of 124 patients with LDH admitted to Nanfeng County Hospital of Traditional Chinese Medicine from January 2019 to December 2021 were randomly divided into two groups,with 62 cases in each group.The control group was treated with lor-noxicam,and the observation group was additionally treated with Chinese medicine fumigation for 10 consecutive days.The clinical ef-ficacy,lumbar function,pain degree,daily living ability,serological indexes and adverse reactions of the two groups were compared.Results The total effective rate of the observation group was 96.77%(60/62),which was higher than that of the control group(85.48%,53/62)(P<0.05).After treatment,the Japanese Orthopaedic Association evaluation treatment score(JOA score),activity of daily living(ADL)score in observation group were higher than those in control group,visual simulation scale(VAS)score,tumor necrosis fact-α(TNF-α)and recombinant human matrix metalloproteinase-3(MMP-3)level in observation group were lower than those in control group(P<0.05).No serious adverse reactions were observed in the two groups.Conclusion Chinese medicine fumigation and lornoxi-cam can reduce inflammatory reaction,improve vertebral function and activities of daily living,and reduce TNF in patients with LDH-α,The level of MMP-3,with significant effect and good safety,and is worthy of clinical promotion.
