针刺疗法联合关节腔注射玻璃酸钠治疗膝骨关节炎对患者疼痛与功能恢复的影响

摘　　要：目的探究针刺疗法联合关节腔注射玻璃酸钠治疗膝骨关节炎(KOA)对患者疼痛与功能恢复的影响。方法选取2019年1月至2020年8月于北京水利医院针灸理疗科门诊接受治疗的80例KOA患者作为研究对象,按照随机数字表法分为注射组与联合组,每组40例。注射组采用单纯关节腔注射玻璃酸钠,联合组在注射组基础上采用针刺治疗。比较两组视觉模拟评分法(VAS)、西大略湖和麦克马斯特大学骨关节指数量表(WOMAC)、生命质量综合评定问卷(GQOL-74)评分。结果治疗后1个月,两组VAS、WOMAC评分比较差异无统计学意义;治疗后6个月,联合组VAS、WOMAC评分均低于注射组,差异有统计学意义(P<0.05)。治疗后6个月,联合组躯体功能、物质功能、社会功能评分均高于注射组,差异有统计学意义(P<0.05);两组心理功能评分比较差异无统计学意义。结论针刺疗法联合关节腔注射玻璃酸钠治疗KOA在近期能改善患者疼痛症状及膝关节功能,提高日常生活能力。Objective To explore the effect of acupuncture treatment combined with intra-articular injection of sodium hyaluronate on pain and functional recovery for the patients with knee osteoarthritis(KOA).Methods A total of 80 patients with KOA who were treated in the Outpatient department of Acupuncture and Physiotherapy of Beijing Shuili Hospital from January 2019 to August 2020 were selected as the research subjects,they were divided into the injection group and the combined group according to the random number table method,with 40 cases in each group.The injection group was treated with simple intra-articular sodium hyaluronate,and the combined group was treated with acupuncture on the basis of the injection group.The scores of visual analogue scale(VAS),Western Ontario and McMaster University(WOMAC)and generic quality of lifeinventory-74(GQOL-74)were compared between the two groups.Results 1 month after treatment,there were no significant differences in VAS and WOMAC scores between the two groups;at 6 months after treatment,the VAS and WOMAC scores in the combined group were lower than those in the injection group,and the differences were statistically significant(P<0.05).At 6 months after treatment,the scores of physical function,material function and social function in the combined group were higher than those in the injection group,and the differences were statistically significant(P<0.05);there was no significant difference in psychological function score between the two groups.Conclusion Acupuncture treatment combined with intraarticular injection of sodium hyaluronate therapy can alleviate pain symptoms,improve knee joint function,and enhance daily living ability in patients with KOA.
