摘  要:目的探讨微创经皮钢板接骨术(MIPPO)联合锁定加压钢板(LCP)治疗四肢骨折的临床效果。方法回顾性分析2018年1月至2022年12月南阳市第一人民医院收治的73例四肢骨折患者的临床资料。按手术方法分为切开复位钢板内固定术(ORIF)组(36例)和MIPPO+LCP组(37例)。ORIF组采用常规ORIF进行治疗,MIPPO+LCP组采用MIPPO联合LCP治疗。比较两组治疗优良率、围术期指标、手术前后48 h疾病相关因子[神经生长因子(NGF)、可溶性血管细胞黏附因子(sVCAM)]水平及并发症发生率。结果MIPPO+LCP组治疗优良率(97.30%,36/37)高于ORIF组(75.00%,27/36),差异有统计学意义(P<0.05)。MIPPO+LCP组术中出血量少于ORIF组,手术操作时间、住院天数、骨折愈合时间短于ORIF组(P<0.05)。术后48 h,MIPPO+LCP组NGF水平高于ORIF组,sVCAM水平低于ORIF组(P<0.05)。MIPPO+LCP组并发症发生率(5.41%,2/37)低于ORIF组(25.00%,9/36),P<0.05。结论MIPPO联合LCP治疗四肢骨折效果显著,能缩短手术操作时长和住院天数,改善疾病相关因子水平,促进骨折愈合,且治疗安全性较高。Objective To investigate the efficacy of minimally invasive percutaneous plate osteosynthesis(MIPPO)combined with locking compression plate(LCP)in the treatment of limb fractures.Methods The clinical data from 73 patients with limb fractures treated in Nanyang First People’s Hospital from January 2018 to December 2022 were retrospectively analyzed.According to the surgical methods,the selected patients were divided into the open reduction internal fixation(ORIF)group(36 cases)and the MIPPO+LCP group(37 cases).The ORIF group was treated by conventional ORIF,and the MIPPO+LCP group was treated by MIPPO combined with LCP.The excellent and good rate of treatment,perioperative indicators,preoperative and 48-hour postoperative levels of disease-related factors,including nerve growth factor(NGF)and soluble vascular cell adhesion factor(sVCAM),and incidence of complications were compared between the two groups.Results The excellent and good rate of the MIPPO+LCP group(97.30%,36/37)was higher than that of the ORIF group(75.00%,27/36),and the difference was statistically significant(P<0.05).The MIPPO+LCP group had less intraoperative bleeding,shorter surgical procedures,shorter hospital stay,and shorter fracture healing time compared with the ORIF group(P<0.05).The NGF level in the MIPPO+LCP group was higher than that in the ORIF group,and the sVCAM level was lower than that in the ORIF group(P<0.05).The incidence of complications in the MIPPO+LCP group(5.41%,2/37)was lower than that in the ORIF group(25.00%,9/36),P<0.05.Conclusions MIPPO combined with LCP is effective in the treatment of limb fractures,which can shorten the duration of operation and hospital stay,improve the levels of disease-related factors,and promote fracture healing,with high safety.
