关节镜滑膜切除术治疗不明原因关节炎引发的腕关节病变的效果观察

摘　　要：目的分析关节镜滑膜切除术治疗不明原因关节炎引发的腕关节病变的临床效果。方法回顾性抽取2021年3月至2023年3月郑州大学医院收治的不明原因关节炎引发的腕关节病变患者82例,按照手术方式分为对照组(49例)与研究组(33例)。对照组采用开放性滑膜切除术,研究组采用关节镜滑膜切除术。比较两组患者手术前后视觉模拟量表(VAS)评分、数字评定量表(NRS)评分、Cooney腕关节评分、腕关节活动度及实验室指标[C反应蛋白(CRP)与降钙素原(PCT)水平。结果术后,两组VAS、NRS评分均低于术前,且研究组VAS、NRS评分低于对照组(P<0.05)]。术后,两组关节活动度与术前相比均显著上升,且研究组关节活动度高于对照组(P<0.05)。Cooney评分系统显示,研究组关节功能优良率高于对照组(P<0.05)。术后,两组CRP、PCT水平均低于术前,且研究组CRP、PCT水平低于对照组(P<0.05)。结论关节镜滑膜切除术可有效改善不明原因关节炎引发腕关节病变患者的疼痛程度以及关节功能,提高关节活动度。Objective To analyze the effect of arthroscopic synovectomy(AS)in the treatment of wrist lesions induced by arthritis of unknown cause.Methods Eighty-two patients who underwent surgery for wrist lesions induced by arthritis of unknown cause in Zhengzhou University Hospital from March 2021 to March 2023 were selected retrospectively.And they were divided into a control group(49 cases)and a study group(33 cases)according to surgical method.The control group received open synovectomy,while the study group was treated by arthroscopic synovectomy.Then the visual analogue scale(VAS)score,numerical rating scale(NRS)score,Cooney wrist function score,wrist range of motion,and laboratory indices,including C-reactive protein(CRP)and procalcitonin(PCT),were compared between the two groups before and after surgery.Results After treatment,the VAS score and NRS score in both groups decreased compared with preoperative scores(P<0.05),and the study group had lower VAS and NRS scores than the control group(P<0.05).After treatment,the range of motions in both groups increased,and the increase in the study group was more notable than that in the control group(P<0.05).According to Cooney wrist function score system,the good excellence rate of the study group was higher than that of the control group(P<0.05).After operation,levels of serum CRP and PCT decreased in both groups,and levels of serum CRP and PCT in the study group were lower than those in the control group(P<0.05).Conclusions AS for patients with wrist lesions induced by arthritis of unknown cause can effectively alleviate the pain,improve the joint function and ameliorate the range of motion.
