摘 要:目的使用功能性近红外光谱(fNIRS)检测技术联合焦虑抑郁量表探讨常频听力正常的急性耳鸣患者大脑额叶皮层功能区变化特征,研究额叶皮层功能变化与焦虑抑郁的相关性。方法用fNIRS检测常频听力正常的耳鸣患者与健康对照组的大脑额叶皮层功能连接情况;用汉密尔顿焦虑量表(HAMA)和汉密尔顿抑郁量表(HAMD)评估耳鸣患者焦虑抑郁程度;分析常频听力正常的急性耳鸣患者大脑额叶皮层功能连接(FC)值与HAMA、HAMD量表得分之间的相关性。结果fNIRS功能连接强度分析显示,常频听力正常的急性耳鸣患者的额叶皮质功能连接强度较对照组明显减弱。fNIRS功能连接强度分析显示,常频听力正常的急性耳鸣患者大脑额叶皮层FC值越小,HAMA、HAMD得分越高。结论常频听力正常的急性耳鸣患者大脑额叶皮层功能降低,并且其额叶皮层功能连接强度与HAMA和HAMD得分之间存在显著的负相关关系。Objective To investigate the characteristics of frontal cortex functional area changes in acute tinnitus patients with normal hearing by functional near-infrared spectroscopy(fNIRS)detection technology combined with anxiety and depression scale,and to study the correlation between frontal cortex function changes and anxiety and depression.Methods fNIRS was used to detect functional connectivity of frontal cortex in tinnitus patients with normal hearing and healthy group,and the degree of anxiety and depression in patients with tinnitus was evaluated by Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD).The correlation between the functional connectivity(FC)values of the frontal cortex and the scores of HAMA and HAHD scales in patients with acute tinnitus and normal hearing was analyzed.Results The analysis of fNIRS functional connection strength showed that the frontal cortex functional connection intensity of patients with acute tinnitus and normal hearing was significantly weakened compared with the control group.The analysis of fNIRS functional connection strength showed that the smaller the FC value of the frontal cortex of the brain in patients with acute tinnitus and normal hearing,the higher the HAMA and HAHD scores.Conclusion The function of the frontal cortex of the brain is reduced in patients with acute tinnitus and normal hearing,and there is a significant negative correlation between the functional connection strength of the frontal cortex and the score of HAMA and HAHD in the tinnitus group.
