摘 要:目的探讨沙格列汀、二甲双胍联合门冬胰岛素治疗2型糖尿病(type 2 diabetes mellitus,T2DM)的疗效。方法选取2022年1月—2024年1月丰县人民医院收治的80例T2DM患者为研究对象,按随机数表法分两组,各40例,所有患者入院后予门冬胰岛素治疗,对照组予二甲双胍口服治疗,观察组予二甲双胍联合沙格列汀治疗。比较两组疗效、糖脂代谢指标、胰岛功能及不良反应。结果治疗后,观察组空腹血糖、糖化血红蛋白、餐后2 h血糖、低密度脂蛋白胆固醇、胆固醇、甘油三酯水平均低于对照组,高密度脂蛋白胆固醇水平高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组胰岛素抵抗指数、空腹胰岛素水平低于对照组,胰岛β细胞功能指数、胰岛素敏感性指数高于对照组,差异有统计学意义(P均<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论沙格列汀、二甲双胍联合门冬胰岛素治疗2型糖尿病疗效可显著改善糖脂代谢,改善胰岛素抵抗。Objective To investigate the efficacy of saxagliptin and metformin combined with insulin aspart in the treatment of type 2 diabetes mellitus(T2DM).Methods A total of 80 T2DM patients admitted to Fengxian People's Hospital from January 2022 to January 2024 were selected as the study objects and divided into two groups according to random number table method,with 40 cases in each group.All patients were treated with insulin aspartate after admission,the control group was treated with oral metformin,and the observation group was treated with metformin combined with saxagliptin.The efficacy,glucose and lipid metabolism indexes,islet function and side effects were compared between the two groups.Results After treatment,fasting plasma glucose,glycated hemoglobin,2 hours postprandial plasma glucose,low density lipoprotein cholesterol,cholesterol and triglyceride levels in observation group were lower than those in control group,and high density lipoprotein cholesterol level was higher than that in control group,and the differences were statistically significant(all P<0.05).After treatment,the insulin resistance index and fasting insulin levels in observation group was lower than that in control group,and the isletβcell function index and insulin sensitivity index were higher than that in control group,and the difference was statistically significant(all P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Saxagliptin and metformin combined with insulin aspart can significantly improve glucose and lipid metabolism and insulin resistance in the treatment of type 2 diabetes mellitus.
