摘 要:目的 分析Ⅱ型呼吸衰竭患者分别采用经鼻高流量氧疗与无创呼吸机辅助通气治疗的效果。方法 简单随机选取2022年1月—2023年12月仪征市人民医院收治的100例Ⅱ型呼吸衰竭患者为研究对象,按照随机数表法分为两组。对照组50例,予无创呼吸机辅助通气治疗,观察组50例,予经鼻高流量氧疗。比较两组临床治疗指标、血气指标、并发症与预后情况。结果 治疗后,两组临床治疗指标比较,差异无统计学意义(P>0.05)。治疗后,两组氧分压、二氧化碳分压、pH值比较,差异无统计学意义(P均>0.05)。观察组并发症发生率(6.00%)低于对照组(22.00%),差异有统计学意义(χ^(2)=5.316,P<0.05)。两组插管率、病死率比较,差异无统计学意义(P均>0.05)。结论 Ⅱ型呼吸衰竭患者采用经鼻高流量氧疗与无创呼吸机辅助通气治疗的疗效相当,均能有效改善患者的呼吸困难症状与血气指标,但经鼻高流量氧疗并发症较少,患者治疗耐受性较好。Objective To analyze the effects of nasal high-flow oxygen therapy and non-invasive ventilator-assisted ventilation in patients with type II respiratory failure.Methods A total of one hundred patients with type II respiratory failure admitted to Yizheng People's Hospital from January 2022 to December 2023 were simple randomly selected as the study objects and divided into two groups according to random number table method.The control group(50 cases)received non-invasive ventilator-assisted ventilation,and the observation group(50 cases)received nasal high-flow oxygen therapy.The clinical therapeutic indexes,blood gas indexes,complications and prognosis of the two groups were compared.Results After treatment,there was no statistically significant difference in clinical treatment indexes between the two groups(P>0.05).After treatment,there was no statistically significant difference in oxygen partial pressure,carbon dioxide partial pressure and pH value between the two groups(all P>0.05).The complication rate of observation group(6.00%)was lower than that of control group(22.00%),and the difference was statistically significant(χ^(2)=5.316,P<0.05).There was no statistically significant difference in intubation rate and mortality between the two groups(both P>0.05).Conclusion In patients with type II respiratory failure,both nasal high-flow oxygen therapy and non-invasive ventilator-assisted ventilation can effectively improve dyspnea symptoms and blood gas indexes of patients, but nasal high-flow oxygen therapy has fewer complications and better treatment tolerance.
