摘 要:目的 探讨老年慢性肾功能衰竭(Chronic Renal Failure, CRF)患者采用左卡尼汀联合血液透析治疗的临床效果。方法 方便选取2021年9月—2023年8月高唐县中医院收治的90例老年CRF患者为研究对象,以随机数表法分成对照组和研究组,每组45例。对照组给予血液透析治疗,研究组给予左卡尼汀联合血液透析治疗,比较两组临床疗效、肾功能[尿素氮(Blood Urea Nitrogen, BUN)、血肌酐(Serum Creatinine, Scr)、内生肌酐清除率(Creatinine Clearance Rate, Ccr)、胱抑素(Cystatin C, CysC)、肾小球滤过率(Estimated Glomerular Filtration Rate, eGFR)]、炎症因子水平[肿瘤坏死因子-α(Tumor Necrosis Factor-α, TNF-α)、C反应蛋白(Creaction Protein, CRP)、白细胞介素-8(Interleukin-8, IL-8)]及毒素清除率。结果 研究组治疗总有效率为91.11%,高于对照组的73.33%,差异有统计学意义(χ^(2)=4.865,P<0.05)。治疗后两组Ccr、eGFR升高,BUN、Scr、CysC降低,且研究组Ccr、eGFR较对照组更高,BUN、Scr、CysC更低,差异有统计学意义(P均<0.05)。治疗后两组TNF-α、CRP、IL-8水平均下降,且研究组较对照组更低,差异有统计学意义(P均<0.05)。研究组血钾、血磷、甲状旁腺激素清除率较对照组更高,差异有统计学意义(P均<0.05)。结论 左卡尼汀联合血液透析治疗老年CRF患者,能够提升临床疗效,改善肾功能,缓解炎症反应,促进毒素清除率提高。Objective To explore the clinical effect of levocarnitine combined with hemodialysis treatment in elderly patients with chronic renal failure(CRF).Methods Ninety cases of elderly CRF patients admitted to Gaotang County Hospital of Traditional Chinese Medicine from September 2021 to August 2023 were conveniently selected for the study,and were divided into the control group and the study group by random number table method,with 45 cases in each group.The control group was given hemodialysis treatment,and the study group was given levocarnitine combined with hemodialysis treatment.Compare the clinical efficacy,renal function[blood urea nitrogen(BUN),serum creatinine(Scr),endogenous creatinine clearance rate(Ccr),cystatin C(CysC),estimated glomerular filtration rate(eGFR)],inflammatory factor levels[tumor necrosis factor-α(TNF-α),C-reaction protein(CRP),interleukin-8(IL-8)],and toxin clearance rate of the two groups.Results The total effective rate of treatment in the study group was 91.11%,which was higher than that of 73.33%in the control group,and the difference was statistically significant(χ^(2)=4.865,P<0.05).Ccr and eGFR increased and BUN,Scr and CysC decreased in both groups after treatment,and Ccr and eGFR were higher and BUN, Scr and CysC were lower in the study group than those in the control group, and the differences were statistically significant (all P<0.05). The levels of TNF-α, CRP, and IL-8 decreased in both groups after treatment, and they were lower in the study group than those in the control group, the differences were statisti cally significant (all P<0.05). Blood potassium, blood phosphorus, and parathyroid hormone clearance were higher in the study group than in the control group, and the differences were statistically significant (all P<0.05). Conclusion Levocarnitine combined with hemodialysis for the treatment of elderly CRF patients can enhance the clinical efficacy, improve renal function, alleviate the inflammatory response, and promote the improvement of toxin clearance.
