摘　　要：目的探讨封闭式负压引流术(VSD)联合胰岛素外敷治疗非哺乳期乳腺炎慢性窦道的效果。方法选取2021年1月至2023年1月赣州市妇幼保健院收治的60例浆细胞性乳腺炎女性患者作为研究对象,按照随机数字表法分为对照组(A组,20例)、生理盐水+VSD组(B组,20例)、胰岛素溶液+VSD组(C组,20例)。其中A组给予纱布填塞和常规引流,B组给予VSD治疗,C组采用VSD联合外用胰岛素治疗。观察三组治疗前后疼痛程度和创面面积,治疗后创面闭合时间、总治疗时间、治疗有效率、乳房毁损率及乳腺炎复发率。结果治疗后,三组患者疼痛程度和创面面积低于本组治疗前,差异有统计学意义(P<0.05);B、C组疼痛程度低于A组,创面面积小于A组,创面闭合时间、总治疗时间短于A组,差异有统计学意义(P<0.05);C组疼痛程度低于B组,创面面积小于B组,创面闭合时间、总治疗时间短于B组,差异有统计学意义(P<0.05);B、C组治疗后治疗有效率高于A组,乳房毁损率和复发率低于A组,差异有统计学意义(P<0.017);C组治疗有效率高于B组,乳房毁损率和复发率低于B组,差异有统计学意义(P<0.017)。结论VSD联合外用胰岛素可促进乳腺炎慢性窦道伤口愈合、缩短治疗时间,提高治疗效果,并减轻疼痛、减少乳房并发症和乳腺炎复发率。Objective To investigate the curative effect of vacuum sealing drainage(VSD)combined with external application of insulin on chronic sinus of non-lactation mastitis.Methods A total of sixty female patients with plasma cell mastitis admitted to Ganzhou Women and Children's Health Care Hospital from January 2021 to January 2023 were selected as the study objects and they were divided into control group(group A,20 cases),normal saline+VSD group(group B,20 cases)and insulin solution+VSD group(group C,20 cases)by random number table method.Group A was treated with gauze packing and routine drainage,group B was treated with VSD,and group C was treated with VSD combined with topical insulin.The pain degree,wound area,wound closure time,total treatment time,treatment effectiveness,breast destruction rate and mastitis recurrence rate were observed.Results The pain degree and wound area of the three groups after treatment were lower than that of those before treatment,and the differences were statistically significant(P<0.05).The pain degree in group B and C was lower than that of group A,the wound area in group B and C was smaller than group A,the wound closure time and total treatment time in group B and C were shorter than those in group A,with statistical significances(P<0.05).The pain degree in group C was lower than that of group B,the wound area in group C was smaller than that of group B,the wound closure time and total treatment time in group C were shorter than those in group B,and the differences were statistically significant(P<0.05).The effective rates of treatment in group B and C were higher than those in group A,while the rates of breast damage and recurrence were lower than those in group A,with statistical significance(P<0.017);The effective rate of treatment in group C was higher than that in group B,and the rates of breast damage and recurrence were lower than those in group B,and the differences were statistically significant(P<0.017).Conclusion VSD combined with topical insulin can promote the healing
