摘　　要：目的探讨孕中期血清中人绒毛膜促性腺激素β(β-HCG)、甲胎蛋白(AFP)对前置胎盘合并胎盘粘连(PPWCPA)的预测价值。方法回顾性分析2022年1月至2023年12月于毕节市妇幼保健院住院行剖宫产的100例产妇的临床资料,根据诊断结果将100例产妇分为三组,其中未发现胎盘病变的35例产妇分为正常对照组,单纯前置胎盘的30例产妇分为对照组,PPWCPA的35例产妇分为观察组。比较三组产妇孕中期的β-HCG、AFP水平,并绘制ROC曲线分析β-HCG、AFP对PPWCPA的预测价值。结果观察组的AFP水平高于对照组及正常对照组,差异有统计学意义(P<0.05);观察组的β-HCG水平高于正常对照组,差异有统计学意义(P<0.05)。当AFP cut-off值为1.29 Mom、β-HCG cut-off值为1.19 Mom时,即可高度怀疑PPWCPA。结论孕中期监测血清中β-HCG和AFP对于PPWCPA的预测价值具有临床意义。Objective To investigate the predictive value of serum human chorionic gonadotropin β(β-HCG)and alpha-fetoprotein(AFP)in the second trimester of pregnancy for placenta previa complicated with placental adhesion(PPWCPA).Methods The clinical data of 100 pregnant women who underwent cesarean section in Bijie Maternal and Child Health Hospital from January 2022 to December 2023 were retrospectively analyzed.According to the diagnosis results,100 pregnant women were divided into three groups,among which 35 pregnant women without placental lesions were divided into normal control group,30 pregnant women with simple placenta previa were divided into control group,and 35 cases of PPWCPA were divided into observation group.The levels of β-HCG and AFP in the second trimester of the three groups were compared,and the ROC curve was drawn to analyze the predictive value of β-HCG and AFP for PPWCPA.Results The AFP level of the observation group was higher than that of the control group and that of the normal control group,and the differences were statistically significant(P<0.05).The β-HCG level of the observation group was higher than that of the normal control group,and the difference was statistically significant(P<0.05).PPWCPA can be highly suspected when the cut-off value of AFP is 1.29 Mom and the cut-off value of β-HCG is 1.19 Mom.Conclusion The monitoring of serum β-HCG and AFP during the second trimester has clinical significance for the prediction value of PPWCPA.
