中药热敷联合预防性干预在老年骨关节炎中的应用效果

摘　　要：目的探讨中药热敷联合预防性干预在老年骨关节炎中的应用效果。方法回顾性选取2019年1月至2022年2月首都医科大学北京安贞医院中医科收治的90例老年骨关节炎患者作为研究对象,按照干预方法的不同分为观察组和对照组,各45例。对照组接受预防性干预,观察组在对照组的基础上联合采用自制中药热敷的方法。比较两组的美国膝关节协会量表(KSS)评分、膝关节活动度(ROM)、美国特种外科医院膝关节评分(HSS)、视觉模拟评分法(VAS)评分、髋-膝-踝夹角(HKA)、胫股角(FTA)。结果观察组干预3、6、12个月的疼痛和缺陷扣分低于对照组,稳定性、活动度、膝关节评分总分、功能评分总分高于对照组,差异有统计学意义(P<0.05)。观察组干预3、6、12个月的ROM、HSS评分高于对照组,VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组干预3、6、12个月的HKA值高于对照组,FTA低于对照组,差异有统计学意义(P<0.05)。结论中药热敷可改善患者的关节功能、缓解痛苦,值得推广。Objective To explore the application effect of traditional Chinese medicine hot compress combined with preventive intervention in senile osteoarthritis.Methods A total of 90 elderly patients with osteoarthritis admitted to the Department of Traditional Chinese Medicine,Beijing Anzhen Hospital,Capital Medical University from January 2019 to February 2022 were retrospectively selected as the study objects,and were divided into observation group and control group according to different intervention methods,with 45 cases in each group.The control group received preventive intervention,while the observation group was combined with the control group to take the method of self-made Chinese medicine hot compress.Keen society score(KSS),range of motion(ROM),hospital for special surgery knee score(HSS),visual analogue scale(VAS)score,hip-knee-ankle angle(HKA)and femor-tibial angle(FTA)were compared between the two groups.Results The pain and defect scores of the observation group at 3,6 and 12 months were lower than those of the control group,and the stability,mobility,total score of knee joint and total score of function score were higher than those of the control group,with statistical significance(P<0.05).At 3,6 and 12 months,the ROM and HSS scores of the observation group were higher than those of the control group,and the VAS scores were lower than those of the control group,with statistical significance(P<0.05).The HKA value of the observation group at 3,6 and 12 months was higher than that of the control group,and the FTA value was lower than that of the control group,the differences were statistically significant(P<0.05).Conclusion Traditional Chinese medicine hot compress can improve joint function and relieve pain of patients,which is worth popularizing.
