左旋甲状腺素治疗妊娠期亚临床甲状腺功能减退症的临床实践

摘　　要：目的对妊娠期亚临床甲状腺功能减退症(以下简称甲减)患者使用左旋甲状腺素治疗后,观察其治疗效果及对妊娠结局的影响。方法2022年1月至2023年12月选取妊娠期亚临床甲减患者98例,使用随机数字表法分为两组,各49例。对照组使用常规甲状腺片治疗,观察组使用左旋甲状腺素治疗。对两组甲状腺激素和叶酸指标进行检测,同时对不良妊娠结局进行详细记录,比较临床治疗效果。结果观察组与对照组治疗总有效率分别为95.92%、81.63%,差异有统计学意义(P<0.05);与对照组相比,观察组血清游离甲状腺素、游离三碘甲状腺原氨酸及叶酸水平均较高,促甲状腺激素水平较低,不良妊娠结局发生率也较低(P<0.05)。结论左旋甲状腺素对妊娠期亚临床甲减患者具有良好治疗作用,可改善其甲状腺功能,减少不良妊娠结局,安全性高,值得大力推广与应用。Objective To observe the effect of levothyroxine on pregnancy outcome after treatment of subclinical hypothyroidism(hypothyroidism)during pregnancy.Methods From January 2022 to December 2023,98 subjects with subclinical hypothyroidism during pregnancy were selected and divided into two groups with 49 cases each using random number table method.The control group was treated with conventional thyroid tablets,and the observation group was treated with levothyroxine.Thyroid hormone and folic acid indexes of the two groups were detected,adverse pregnancy outcomes were recorded in detail,and the clinical therapeutic effect was compared.Results The total effective rate of the observation group and the control group was 95.92%and 81.63%,with statistical significance(P<0.05);Compared with the control group,the levels of serum free thyroxine,free triiodothyronine and folate in the observation group were higher,the levels of thyroid stimulating hormone were lower,and the incidence of adverse pregnancy outcomes was also lower(P<0.05).Conclusion Levothyroxine has a good therapeutic effect on subclinical hypothyroidism in pregnancy,can improve thyroid function and reduce adverse pregnancy outcomes,which is safe and worthy of promotion and application.
