脉络学说营卫理论指导针刺处方治疗脑血管疾病

摘　　要：目前脑血管病成为我国受影响人数最多的疾病,吴以岭院士脉络学说之营卫理论揭示了缺血性脑血管疾病病机“凝”→“壅”→“塞”→“不通”的传变规律,为缺血性脑血管疾病的治疗提供了新的思路。根据脉络学说之营卫理论进行辨证选穴,其治疗干预的核心为调和营卫气血、疏通络脉。针刺处方中合谷、足三里调和营卫、化瘀通络,三阴交、血海行气活血,内关、百会活血化瘀、开窍醒神,这些穴位组合能够在不同环节、不同层面阻断缺血性脑血管疾病的级联反应,体现了中医学“控上游因素,治当前病变,防下游传变”的“治未病”思想。At present,cerebrovascular diseases have become the most affected disease in China.Academician Wu Yiling′s theory of nutritive and defensive Qi of meridian theory reveals the transmission law of“coagulation”→“stoppage”→“blockage”→“obstruction”in the pathogenesis of ischemic cerebrovascular diseases,which provides a new idea for the treatment of ischemic cerebrovascular diseases.According to the theory of nutritive and defensive Qi of meridian theory,acupoints are selected based on syndrome differentiation.The core of therapeutic intervention is to regulate nutritive and defensive Qi and blood and dredge collaterals.Acupuncture prescription includes Hegu and Zusanli to regulate nutritive and defensive Qi,dissipate blood stasis and dredge collaterals,Sanyinjiao and Xuehai to activate Qi and blood circulation,and Neiguan and Baihui to activate blood circulation and dissipate blood stasis,open the orifices and awaken the mind.The combination of these acupoints can block the cascade reaction of ischemic cerebrovascular diseases in different links and levels,and promote the idea of“preventive treatment of disease”in traditional Chinese medicine of“controlling upstream factors,treating current diseases,and preventing downstream transmission”.
