舒筋活血汤辅助髋关节置换术治疗老年股骨颈骨折临床研究

摘　　要：目的:观察舒筋活血汤辅助髋关节置换术治疗老年股骨颈骨折的临床疗效。方法:选取128例老年股骨颈骨折患者,以随机数字表法分为观察组和对照组各64例。治疗期间,观察组、对照组均剔除2例,2组均有62例完成研究。2组均行髋关节置换术治疗。对照组术后给予西药治疗,并做好切口换药等护理工作。观察组在对照组基础上给予舒筋活血汤治疗。比较2组临床疗效、Harris评分、血液流变学指标及并发症发生率。结果:术后第35天,观察组总有效率96.77%,高于对照组87.10%,差异有统计学意义(P<0.05)。2组Harris评分均较术后第1天升高,观察组Harris评分高于对照组,差异均有统计学意义(P<0.05)。2组红细胞聚集指数、血浆黏度、全血低切黏度、全血高切黏度、纤维蛋白原水平均较术后第1天降低,观察组上述5项指标水平均低于对照组,差异均有统计学意义(P<0.05)。术后35 d内,观察组并发症发生率6.45%,低于对照组19.35%,差异有统计学意义(P<0.05)。结论:舒筋活血汤辅助髋关节置换术治疗老年股骨颈骨折疗效显著,对髋关节功能及血液流变学指标均有改善作用,并能减少术后并发症发生。Objective:To observe the clinical effect of Shujin Huoxue Decoction assisting hip replacement for femoral neck fracture in senile patients.Methods:A total of 128 cases of senile patients with femoral neck fracture were selected and divided into the observation group and the control group according to the random number table method,with 64 cases in each group.During the treatment period,two cases were excluded respectively from the observation group and the control group,and 62 cases were completed in each group.Both groups underwent hip replacement surgery.The control group was treated with western medicine after surgery,and nursing work such as incision dressing change was carried out.The observation group was treated with Shujin Huoxue Decoction based on the treatment of the control group.The clinical effects,Harris scores,hemorheological indexes,and incidence of complications were compared between the two groups.Results:On the day 35 after surgery,the total effective rate was 96.77%in the observation group,higher than that of 87.10%in the control group,the difference being significant(P<0.05).The Harris scores in the two groups were increased when compared with those on the first day after surgery,and the Harris score in the observation group in the control group,differences being significant(P<0.05).The levels of red blood cell aggregation index,plasma viscosity,whole blood low shear viscosity,whole blood high shear viscosity,and fibrinogen in the two groups were decreased when compared with those on the first day after surgery.The levels of the above five indexes in the observation group were lower than those in the control group,differences being significant(P<0.05).Within 35 days of surgery,the incidence of complications was 6.45%in the observation group,lower than that of 19.35%in the control group,the difference being significant(P<0.05).Conclusion:Shujin Huoxue Decoction assisting hip replacement has a significant curative effect in treating femoral neck fracture in senile patients,which can improve hip
