摘 要:冠状动脉易损斑块容易破裂,导致冠状动脉血栓性阻塞,是引发主要心血管不良事件的重要致病机制。斑块破裂和斑块侵蚀是易损斑块的两种主要表型,具有不同的临床特点和治疗策略。斑块侵蚀患者所引起的心肌梗死范围较小,预后较好。多模式血管内影像学技术的整合应用有助于评估斑块不稳定性,提高诊断准确度和治疗效果。本文就近年来冠状动脉易损斑块的临床研究进展进行综述。Vulnerable plaques in the coronary arteries are prone to rupture,leading to thrombotic obstruction of the coronary artery,which is a significant pathogenic mechanism of major adverse cardiovascular events.Vulnerable plaques can manifest as two main phenotypes:plaque rupture and plaque erosion,each having distinct clinical characteristics and treatment strategies.Patients with plaque erosion tend to have smaller myocardial infarct size and better prognosis.The integration of multimodality intravascular imaging techniques helps in assessing plaque instability,thereby improving diagnostic accuracy and treatment efficacy.This article provides a comprehensive review of the clinical research advancements in vulnerable plaques in recent years.
