摘 要:目的 评估血清促红细胞生成素(EPO)、高迁移率族蛋白B1(HMGB1)及葡萄糖调节蛋白78(GRP78)与烧伤后并发脓毒症患者的相关性。方法 选取于徐州医科大学附属宿迁医院2021年1月—2021年12月就诊的烧伤患者作为研究对象,共计110例。根据感染程度分为烧伤后脓毒症组21例、局部感染组47例、未感染组42例。收集患者烧伤面积、糖尿病、高血压患病情况等一般资料。收集外周静脉血,留血清样本采用ELISA法检测EPO、HMGB1及GRP78水平。采用受试者工作曲线(ROC)评估EPO、HMGB1、GRP78对烧伤患者并发脓毒症的预测价值。结果 在烧伤面积方面,局部感染组大于未感染组,而脓毒症组大于局部感染组,差异有统计学意义(P<0.05)。在血清EPO、HMGB1及GRP78水平方面,局部感染组高于未感染组,而脓毒症组高于局部感染组,差异有统计学意义(P<0.05)。EPO预测烧伤患者发生脓毒症的敏感度为76.2%,特异度为80.9%;HMGB1预测烧伤患者发生脓毒症的敏感度为71.4%,特异度为84.3%;GRP78预测烧伤患者发生脓毒症的敏感度为71.4%,特异度为89.9%。结论 烧伤后合并脓毒症患者血清EPO、HMGB1及GRP78水平明显升高,血清EPO、HMGB1及GRP78均对烧伤患者并发脓毒症有较高的预测价值。Objective To evaluate the correlation of serum EPO,HMGB1 and GRP78 with sepsis after burn.Methods A total of 110 burn patients admitted from January 2021 to December 2021 were selected as research objects.According to whether there was concurrent infection,the patients were divided into 21 cases of post-burn sepsis,47cases of local infection and 42 cases of non-infection.The patients with local infection after burn were diagnosed by wound tissue culture results,and sepsis met the diagnostic criteria for burn sepsis in the Diagnostic Criteria and Treatment Guidelines for Burn Infection.General data such as age,sex,BMI and burn area were collected.Venous blood was collected,and serum samples were retained for detection of EPO,HMGB1 and GRP78 levels by ELISA.Results In terms of burn area,the local infection group was larger than the uninfected group,and the sepsis group was larger than the local infection group,the difference was statistically significant(P<0.05).In terms of EPO,HMGB1 and GRP78 level,the local infection group was higher than the uninfected group,and the sepsis group was higher than the local infection group,the difference was statistically significant(P<0.05).In predicting sepsis in burn patients,the sensitivity of EPO was 76.2%,the specificity of EPO was 80.9%;the sensitivity of HMGB1 was 71.4%,the specificity of HMGB1 was 84.3%.the sensitivity of GRP78 was 71.4%,the specificity of GRP78 was 89.9%.Conclusion Serum levels of EPO,HMGB1 and GRP78 in burn patients complicated with sepsis were significantly increased,and serum EPO,HMGB1 and GRP78 had high predictive value for burn patients complicated with sepsis.
