米索前列醇联合芬太尼、丙泊酚应用于绝经后宫内节育器取出术的效果分析

摘　　要：目的:探讨米索前列醇联合芬太尼、丙泊酚,在绝经妇女宫内节育器(IUD)取出术中的应用效果.方法:选取2022年1月至2022年12月于本院就诊并进行IUD取出术的80例绝经妇女作为研究对象.按照随机数字表法,将其分为观察组(n=40,术前采用米索前列醇联合芬太尼与丙泊酚处理)和常规组(n=40,术前采用利多卡因处理).比较两组妇女宫颈软化率、术中指标、IUD取出成功率及不良反应率.结果:观察组妇女宫颈软化总有效率为95%(34/40),高于常规组的72.5%(29/40),差异有统计学意义(P<0.05).观察组妇女手术时间短于常规组,术中出血量少于常规组,疼痛评分低于常规组,差异均有统计学意义(P<0.05).观察组妇女IUD取出成功率高于对照组,差异有统计学意义(P<0.05).观察组妇女不良反应率与常规组比较,差异无统计学意义(P>0.05).结论:米索前列醇联合芬太尼、丙泊酚,可以明显改善绝经妇女的宫颈软化效果,提高IUD取出成功率,而且是一种安全性较高的方法,值得临床推广应用.Objective:To explore the application effect of misoprostol combined with fentanyl and propofol in the removal of intrauterine devices(IUD)in postmenopausal women.Methods:Eighty postmenopausal women who underwent IUD removal surgery in our hospital from January 2022 to December 2022 were selected as research subjects.According to the random number table method,they were divided into observation group(n=40,treated with misoprostol combined with fentanyl and propofol before surgery)and conventional group(n=40,treated with lidocaine before surgery).The effective rate of cervical softening,intraoperative indicators,success rate of IUD removal,and the incidence of adverse reaction were compared between the two groups.Results:The total effective rate of cervical softening in observation group was 95%(38/40),which was higher than 72.5%(29/40)in conventional group,and the difference was statistically significant(P<0.05).Observation group had shorter surgical time,less intraoperative bleeding,and lower pain scores than conventional group,with statistically significant differences(P<0.05).The success rate of IUD removal in observation group was higher than in conventional group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The combination of misoprostol,fentanyl,and propofol can significantly improve the cervical softening effect of postmenopausal women,increase the success rate of IUD removal,and is a highly safe method,which is worthy of clinical promotion and application.
