膝关节镜下射频治疗前交叉韧带黏液样变性的疗效分析

摘　　要：目的探讨膝关节镜下射频治疗前交叉韧带黏液样变性(MD-ACL)的临床疗效。方法回顾性分析2018年1月至2021年12月我院收治的30例MD-ACL病人的临床资料。所有病人均行膝关节镜手术,术后进行康复训练。随访时评估疼痛视觉模拟量表(VAS)评分、膝关节功能Lysholm评分和国际膝关节文献委员会(IKDC)评分。结果30例病人均顺利完成手术,手术时间(82.16±15.12)min(60~110 min);术中出血量(5.40±2.49)mL(3~10 mL);未见明显并发症发生。有2例病人行自体肌腱移植重建前交叉韧带术。术后随访无复发迹象,膝关节活动度正常。术前VAS评分为(6.46±1.04)分,术后3个月时降至(3.00±0.78)分,6个月随访时降至(0.56±0.50)分;术前Lysholm评分为(58.50±7.32)分,术后3个月时增加至(74.83±4.25)分,6个月随访时为(87.66±2.85)分;术前IKDC评分为(54.16±4.25)分,术后3个月时增加到(76.13±5.01)分,6个月随访时增加到(87.43±5.88)分;手术前后比较,差异均有统计学意义(P<0.05)。结论膝关节镜下射频修整治疗MD-ACL疗效显著,可明显改善膝关节功能,减轻疼痛,是一种安全、有效的治疗方法。Objective To investigate the clinical efficacy of knee arthroscopic radiofrequency in the treatment of mucoid degeneration of the anterior cruciate ligament(MD⁃ACL).Methods The clinical data of 30 patients with MD⁃ACL treated in our hospital from January 2018 to December 2021 were retrospectively analyzed.All patients underwent knee arthroscopic surgery and rehabilitation training after surgery.Vision analogue scale(VAS)score,Lysholm score,and International Knee Documentation Committee(IKDC)score were assessed during the follow⁃up period.Results All 30 patients successfully completed the operation,and the surgery duration was(82.16±15.12)min(60⁃110 min).Intraoperative blood loss was(5.40±2.49)mL(3⁃10 mL).No complications occurred.There were 2 patients undergoing autologous graftanterior cruciate ligament reconstruct.There was no sign of recurrence during the postoperative follow⁃up period,and the knee range of motion was normal.The VAS score was 6.46±1.04 before surgery,decreased to 3.00±0.78 at 3rd month,and 0.56±0.50 at 6th month during the follow⁃up period.The preoperative Lysholm score was 58.50±7.32,which increased to 74.83±4.25 at 3rd month,and 87.66±2.85 at 6th month during the follow⁃up period.The IKDC score was 54.16±4.25 before operation,which increased to 76.13±5.01 at 3rd month and 87.43±5.88 at 6th month during the follow⁃up period.The differences before and after surgery were statistically significant(P<0.05).Conclusion Knee arthroscopic radiofrequency therapy is a safe and effective treatment for MD⁃ACL,which can significantly improve knee joint function and reduce pain.
