摘　　要：目的 分析重症脓毒症患者血栓中心静脉导管相关性深静脉血栓(Central venous catheter-associated deep venous thromboses,CADVT)发生现况及其影响因素。方法 选择2023年1-12月在新疆地区某三甲医院重症医学科进行中心静脉置管的脓毒症患者为研究对象,以是否发生CADVT分为血栓组、非血栓组,比较两组患者的人口学资料、置管资料及检验指标。采用Logistic回归分析重症脓毒症患者CADVT的影响因素。结果 共纳入504例中心静脉置管的脓毒症患者,其中52例发生CADVT,发生率为10.32%。Logistic回归分析显示:吸烟史、假体植入史、置管穿刺次数、D-二聚体升高、致病菌为革兰阴性菌是脓毒症患者CADVT发生的独立影响因素(P<0.05)。结论 吸烟史、假体植入史、置管穿刺次数、D-二聚体升高、致病菌为革兰阴性菌是脓毒症患者CADVT发生的独立影响因素,针对此类患者应更加关注其血栓发生情况。Objective To analyze the present situation and influencing factors of central venous catheter-related deep venous thrombosis in sepsis patients.Methods From Jan 2023 to Dec 2023,sepsis patients with central venous catheter were studied in the hospital in Xinjiang.The patients were divided into 2 groups and the demographic and clinical data of the 2 groups were compared.The patients were divided into thrombus group and non-thrombus group according to whether central venous catheter-related deep venous thrombosis occurred.Logistic regression was used to analyze the risk factors and explored the influencing factors of central venous catheter-related deep venous thrombosis in sepsis patients.Results In the study,504 patients were included,52 patients developed central venous catheter-related deep venous thrombosis(10.32%).Logistic regression analysis showed that smoking history,prosthesis implantation history,number of catheterization,D-dimer elevation and Gram-negative bacteria were independent influencing factors for the occurrence of central venous catheter-related deep venous thrombosis in spesis patients(P<0.05).Conclusion Smoking history,prosthesis implantation history,times of catheterization,D-dimer elevation and Gram-negative bacteriawere are independent influencing factors for the occurrence of central venous catheter-related deep venous thrombosis in spesis patients,and more attention should be paid to these patients.
