摘　　要：目的研究关节滑膜中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)含量与软骨下骨髓水肿(BME)程度及退变性膝关节骨性关节炎(KOA)的相关性。方法选择2022年4月至2023年3月医院收治的30例伴BME退变性KOA住院患者,作为实验组;选择同期住院的10例伴BME非退变性KOA患者,作为对照组。两组均给予关节滑膜TNF-α、IL-6检测,分析TNF-α、IL-6含量对退变性KOA形成与发展、BME程度及膝关节功能的影响。结果入院时,实验组美国膝关节协会评分(AKS)评分低于对照组,兴趣区面积(ROIA)测定量高于对照组,差异有统计学意义(P<0.05)。实验组术中关节滑膜组织中TNF-α、IL-6含量均高于对照组,差异有统计学意义(P<0.05)。Pearson相关性分析显示,患者入院时的AKS评分与术中TNF-α、IL-6含量均呈负相关关系(P<0.05);患者入院时测定BME的ROIA值与术中TNF-α、IL-6含量均呈正相关关系(P<0.05)。结论退变性KOA患者关节滑膜中TNF-α、IL-6的含量与其发生BME的程度存在相关性,并与患者的膝关节功能相关。Objective:To study the correlation between the contents of tumor necrosis factor-α(TNF-α)and inter-leukin-6(IL-6)in synovium of joint and the degree of subchondral bone marrow edema(BME)and degenerative osteoarthritis of knee joint(KOA).Methods:From April 2022 to March 2023,30 inpatients with KOA with BME degeneration were selected as the experimental group.10 patients with non-degenerative KOA with BME who were hospitalized in the same period were selected as the control group.TNF-αand IL-6 in synovium of joints were detected in both groups,and the effects of TNF-αand IL-6 content on the formation and development of de-generative KOA,BME degree and knee joint function were analyzed.Results:At the time of admission,the score of American Knee Joint Association(AKS)in the experimental group was lower than that in the control group,and the area of interest(ROIA)was higher than that in the control group,with statistical significance(P<0.05).The con-tents of TNF-αand IL-6 in synovial tissue in experimental group were higher than those in control group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that the AKS score at ad-mission was negatively correlated with the contents of TNF-αand IL-6 during operation(P<0.05).The ROIA value of BME was positively correlated with the contents of TNF-αand IL-6 during operation(P<0.05).Conclu-sion:For patients with degenerative KOA,the contents of TNF-αand IL-6 in synovium of joint are related to the degree of BME and the function of knee joint.
